利多卡因硬膜外阻滞对CCI模型大鼠脊髓c-fos表达的影响 被引量:1

EFFECT OF EPIDURAL NERVE BLOCK WITH LIDOCAINE ON THE SPINE CORD EXPRESSION OF C-FOS IN A RAT MODEL OF CHRONIC CONSTRICTION NERVE INJURY

摘要 目的:观察利多卡因硬膜外阻滞后神经病理性疼痛大鼠行为学和热痛阈的变化及对脊髓后角c-fos表达的影响。方法:雌性Wistar大鼠24只,随机分为三组,每组8只(n=8)。(1)假手术组(S组):右侧坐骨神经只暴露5分钟,未结扎,术后第4天起硬膜外注入生理盐水0.1ml,每日1次,连续10天;(2)对照组(C组):右侧坐骨神经结扎,术后第4天起硬膜外注入生理盐水0.1ml,每日1次,连续10天;(3)利多卡因组(L组):右侧坐骨神经结扎,术后第4天起硬膜外注入1%利多卡因0.1ml,每日1次,连续10天。分别于术后第1,4,7,10,14天观察行为学变化。于术前及术后第4,7,10,14天用JL-F数显式光热测痛仪测定双后肢热痛阈。术后第14天处死大鼠取出腰段脊髓,石蜡包埋,病理切片,免疫组化法检测c-fos免疫阳性细胞(FLI)。结果:(1)C组、L组与S组比较术后下肢运动评分均有非常显著差异(P<0.01),C组与L组之间无显著差异(P>0.05)。(2)C组、L组与S组比较各实验点热痛阈均有显著差异(P<0.05或P<0.01);L组与C组间各实验点相比差异无显著意义(P>0.05)。(3)C组、L组与S组相比,各层FLI细胞数差异均具有显著性意义(P<0.05或P<0.01);L组各层FLI细胞数与C组相比无显著差异(P>0.05)。结论:应用利多卡因进行硬膜外阻滞对CCI模型大鼠脊髓c-fos的表达抑制不明显。 Objective: To observe the changes of behavior and thermal pain threshold in a rat model of neuropathic pain after epidural nerve block with lidocaine,and its effect on the expression of c-fos in spinal cord.Methods: 24 female Wistar rats with body weights of 220~280 grams were randomized into three groups,8 for each group.(1) Sham operation group(group S): right sciatic nerves of rats were exposed for 5 minutes but not ligated in this group.Since the 4th day after operation,0.1ml saline was injected into epidural space each day for the next 10 days.(2) Control group(group C): right sciatic nerves were ligated and 0.1ml saline was injected daily since the 4th day after operation in group S.(3) Lidocaine group(group L): right sciatic nerves were ligated and 0.1ml 1% lidocaine was injected into epidural space each day for the next 10 day.Rat behavior was observeds on the 1st,4th,7th,10th and 14th day after the operation.Thermal pain threshold of both hind legs was measured with JL-F Digital Display Photothermal Algometer.On the 14th day,rats were executed and lumbar spinal cords were sampled for detecting Fos-like immuno-reactivity cells(FLI) by immunohistochemical assay.Results:(1) There was significant difference(P〈0.01) in post operational motor scores of hind legs between group S and group C and L.(2) There was also significant difference(P〈0.05 or 0.01) in thermal pain threshold at each time point between group S and group C and L.There was no difference between group L and group C(P〉0.05).(3) FLI cell counts in all layers of group C and L were significant different from group S(P〈0.05 or 0.01),but there were no significant difference between group L and group C(P〉0.05).Conclusions: Epidural nerve block with lidocaine did not significantly inhibit the spinal cord expression of c-fos in a rat model of chronic constrinction nerve injury.
