高磷饮食对5/6肾切除大鼠继发性甲状旁腺功能亢进及肾脏纤维化的影响及金雀异黄素干预作用 被引量:2

摘要 目的观察高磷饮食对5/6肾切除大鼠继发性甲状旁腺功能亢进、肾脏纤维化的影响及金雀异黄素的干预作用。方法 8~10周龄雄性Sprague-Dawley大鼠24只,体重200 250 g,随机分为4组,每组6只:假手术正常磷饮食组(Sham十NP组),5/6肾切除正常磷饮食组(STNx+NP组),5/6肾切除高磷饮食组(STNx+HP组),5/6肾切除高磷饮食+金雀异黄素灌胃组(STNx+HP+Gen组)。高磷饮食配方:磷1.2%,钙1.6%,维生素D 1 U/kg。正常磷饮食配方:磷0.6%,钙0.6%,维生素D 1 U/kg。5/6肾脏切除分两步进行。造模术后第4、8、12、16周,检测血清肌酐(SCr)、磷、钙、1,25(OH)_2D_3、全段甲状旁腺激素(iPTH)水平。特定饮食结束时,处死大鼠,取肾脏组织,石蜡切片行苏木精-伊红(H-E)染色,进行病理积分。结果 Sham+NP组各项指标在实验过程中均无明显变化(P值均>0.05)。STNx+NP组、STNx+HP组、STNx+HP+Gen组SCr、磷、iPTH的水平均随着特定饮食喂养时间的延长逐渐升高。16周时与4周时的差异均有统计学意义(P值均<0.01)。16周时,STNx+HP组、STNx+HP+Gen组的SCr、磷、iPTH与STNx+NP组的差异均有统计学意义(P值均<0.01)。而4组不同时间钙、1,25(OH)_2D_3的水平均无明显改变(P值均>0.05)。Sham+NP组、STNx+NP组、STNx+HP组和STNx+HP+Gen组的肾小球硬化指数分别为0、1.01±0.02、1.83±0.03、1.51±0.03,STNx+HP组和STNx+HP+Gen组显著高于STNx+NP组(P值均<0.01),STNx+HP组显著高于STNx+HP+Gen组(P<0.01)。结论高磷饮食可促进5/6肾大部切除大鼠甲状旁腺功能亢进的发生,并加剧其肾功能恶化,金雀异黄素可部分减轻其肾脏毒性作用,改善甲状旁腺功能亢进。 Objective To study the effect of high phosphorus diet on secondary hyperparathyroidism and renal fibrosis in 5/6 nephrectomized rats and its intervention with Genistein.Methods A total of 24 male Sprague-Dawley rats,weighing 200-250 g,aged 8-10 weeks,were randomly divided into 4 groups:sham operation plus normal phosphorus food group(Sham + NP),5/6 nephrectomy plus normal phosphorus food group (STNx+NP),5/6 nephrectomy plus high phosphorus food group(STNx + HP),5/6 nephrectomy plus high phosphorus food group and Genstein(30μg·kg^(-1)·d^(-1)) intragastric administration group(STNx + HP + Gen). The high phosphorus diet contained 1.2%inorganic phosphate,1.6%calcium and 1 U/kg vitamin D.The standard rodent diet included 0.6%inorganic phosphate,0.6%calcium and 1 U/kg vitamin D.Nephrectomy was performed in 2 steps.Serum creatinine(SCr),phosphorus(P),calcium(Ca),1,25(OH)_2D_3 and intact parathormohe (iPTH) levels were determined at week 4,8,12,16 after operation.The renal specimens were obtained and were subjected to pathological analysis at the end of experiment.Results The parameters were similar throughout the experiment in sham+ NP group(P0.05).The levels of SCr,P and iPTH gradually increased along with time in the three STNx groups,with significant difference found between the 16^(th) week and the 4^(th) week (P0.01).At week 16,the levels of SCr,P and iPTH in STnX+HP group and STNx + HP + Gen group were significantly different from those of STNx + NP group(P0.01).The levels of Ca and 1,25(OH)_2D_3 were not significantly different in all the three STNx groups.The score of glumerulus sclerosis was 0 in sham + NP group,the highest score was 1.83±0.03 in STNx + HP group,and 1.51±0.03 in STNx + HP + Gen group,with the latter two significantly higher than that of the STNx + NP group(1.01±0.02),and that of STNx + HP group was significantly higher than that of STNx + HP + Gen group(P0.01).Conclusion Nephrectomy plus high phosphorus foods can induce secondary hyperparathyroidism and aggravate the damage of renal function.Genstein can protect renal damage induced by high phosphorus diet and improve hyperparathyroidism.
