摘要
目的 探讨人体代谢亚硝胺的重要同功酶细胞色素 P45 0 2 E1( CYP2 E1)基因多态性与胃癌危险性的关系。 方法 应用病例对照分子流行病学研究方法 ,采用聚合酶链和限制性片段长度多态性技术 ,对福建省胃癌高发区长乐市 91例原发性胃癌病例和 94例人群对照的 DNA进行CYP2 E1基因型检测和分析。 结果 CYP2 E1的 C1/ C2和 C2 / C2基因型频率在胃癌病例和对照中分别为 36.2 6%和 2 4.47% ,Fisher's精确概率检验 χ2 =5 .344 ,P=0 .0 69。胃癌病例中罕见纯合子 C2 / C2基因型 6例 ( 6.5 9% ) ,而对照中仅 1例 ( 1.0 6% )。胃癌病例组 CYP2 E1的 C2等位基因频率显著高于对照组 ( OR=1.86,95 % CI=1.0 7～ 3.2 5 )。携有 CYP2 E1非 C1/ C1基因型者若饮酒或长期食用鱼露 ,发生胃癌的危险性明显增大。 结论 携带 CYP2 E1的 C2 / C2基因型的个体可能与胃癌的高风险有关。
Genetic polymorphisms in enzymes involved in carcinogen metabolism have been found to influence susceptibility to cancer The objective of this study is to investigate the relationship between Cytochrome P450 2E1(CYP2E1) polymorphism and susceptibility to gastric cancer \ Methods\ After extraction of DNA from peripheral white cells, polymerase chain reaction restriction fragment length polymophism(PCR RFLP) was used to detect CYP2E1 genotype in cases with gastric cancer(n=91) and matched controls(n=94) from a high risk area, Changle, Fujian Alleles C1 and C2 of CYP2E1 were identified by RFLP with endonuclease RsaI or PstI The genotypes were separated into threes types: type A, type C(homozygous for C1 or C2 gene) and type B(heterozygous for both genes) The frequency of genotypes with C2 allele was compared with that of genotypes without C2 among gastric cancer patients The genotype frequencies of CYP2E1 were calculated with Hardy Weinberg equilibrium formula \ Results\ The frequencies of allele C1 and C2 in cancer cases were significantly different from those in controls(χ\+2=4 91,P<0 05) Persons with exposure to fish sauce and allele C2 of CYP2E1 had 18 58 fold risk in developing gastric cancer(OR=18 58, 95%CI:6 47~53 37) \ Conclusion\ These results suggest that gastric cancer risk associated with exposure to fish sauce may be more striking in individuals who carried the C2 allele of CYP2E1 \;
