摘要
目的探讨成人甲型H1N1流感患者血清酶活性和C反应蛋白(CRP)的水平的临床意义。方法检测63例成人甲型H1N1流感患者血清酶活性和CRP水平,按是否并发肺炎将其分为肺炎组、普通组,同时采用31例健康体检者作为对照组,分析各指标与病情的关联性。结果成人甲型H1N1流感患者的血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、乳酸脱氢酶(LDH)、磷酸肌酸激酶(CK)、磷酸肌酸同工酶(CK-MB)和CRP水平,并发肺炎组(18例)较普通组(45例)均明显升高,差异具有统计学意义(P<0.01);普通组血清酶活性均正常,但CRP有轻度升高。结论成人甲型H1N1流感并发肺炎患者常伴随着心、肝等多器官损害,合并细菌感染是除病毒感染外导致肺炎的另一诱因。普通型患者重要脏器损害不明显,炎症反应较轻。
Objective To observe the clinical significance of the serum enzymes activity and CRP level in adult patients with type A H1N1 influenza.Methods 63 adult patients with type A H1N1 influenza were divided into pneumonia group（18 cases） and ordinary group（45 cases） according to whether concurrent pneumonia or not,meanwhile 31 cases of healthy check-up person were collected as control group.The serum enzymes activity and CRP level were detected in all patients and the correlation of index and illness were analyzed.Results The level of serum alanine aminotransferase（ALT）,aspartate aminotransferase（AST）,lactate dehydrogenase（LDH）,creatine kinase（CK）,creatine phosphate isoenzyme（CK-MB） and CRP were increased significantly in pneumonia group compared to ordinary group（P0.01）.The serum enzymes activity was normal,but the CRP was mildly elevated in ordinary group.Conclusion Type A H1N1 influenza patients complicated with pneumonia is often associated with multiple organ damage including heart,liver and so on.Bacterium infection is another cause of pneumonia except virus.The damage of vital organs is not apparent and the inflammatory reaction is lighter in ordinary patients.
