摘要
目的探讨2型糖尿病患者肾病与视网膜病变的共同患病危险因素及二者的相关关系。方法对198例2型糖尿病患者行荧光素眼底血管造影,根据结果分为无糖尿病视网膜病变组、非增殖性糖尿病视网膜病变组、增殖性糖尿病视网膜病变组。根据国际通用的Mongensen分期标准将2型糖尿病患者分为单纯糖尿病组、早期糖尿病肾病组和临床糖尿病肾病组。统计学分析采用SPSS 11.5进行处理,尿微量白蛋白与糖尿病视网膜病变的分析用Logistic多元逐步回归分析。结果尿蛋白组患者发生视网膜病变的比例明显高于单纯糖尿病组,临床糖尿病肾病组合并增殖性视网膜病变的概率明显高于单纯糖尿病组和微量白蛋白尿组。白蛋白尿和糖尿病病程是糖尿病视网膜病变的独立预测因素。结论糖尿病肾病与糖尿病视网膜病变具有相关一致性,但糖尿病肾病分期及糖尿病视网膜病变分级之间无对应关系。
Objective To evaluate the common risk factors of the diabetic nephropathy and the diabetic retinopathy, and to explore the potential relationship between diabetic nephropathy and diabetic retinopathy in type 2 diabetes mellitus patients. Methods 198 eases of type 2 diabetic patients were all confirmed, they all were taking fluorescent fundus angiography （FFA） by the same ophthalmologisL according to the founding of FFA, They were divided into the non - diabetic retinopathy （NDR） group, the non- proliferative diabetic retinopathy （NPDR）group and the proliferative diabetic ritinopathy （PDR） group. And according to the universal Mongensen standard for division, all the T2DM patients were divided into the simple di- abetes mellitus（SDM） group, the early diabetic nephropathy （EDN） group and the clinical diabetic nephropathy （CDN） group. The statistic analysis was performed by SPSS for windons. 11.5, Urinary albumin analysis of diabetic retinopathy with Logistic stepwise regression analysis. Results The oecurance rate of DR in DN group was higher than that of the SDM groups. the occurance rate of PDR was higher in dominant urinary albumin excretion group than that in the SDM group and the mini - urinary albumin group. Conclusion There is positively correlation between the diabetic nephropathy and the diabetic retinopathy, especially with the progression of diabetes, its relevance was more significant. However, stages of diabetic nephropathy and diabetic retinopathy was no correspondence between the classification.
