原发性高血压患者醛固酮合酶基因-344T/C多态性与心房颤动的相关性研究 被引量:7

摘要 目的探讨中国河南豫北地区原发性高血压人群醛固酮合酶(CYP11B2)基因-344T/C多态性与心房颤动(房颤)的关系。方法运用病例对照法,选择803例原发性高血压患者,运用PCR—RFLP技术对405例伴房颤者(房颤组)和398例窦性心律者(窦律组)的CYP11B2基因-344T/C多态性进行基因分型,并进行分析。结果2组T和C等位基因分布,差异无统计学意义(x2=1.32,P=0.25);房颤组CC基因频率明显高于窦律组(x2=21.29,P〈0.05)。与携带TT或TC基因型者比较,携带CC基因型高血压患者患房颤的风险增加(OR=1.96,95%CI:1.23~2.67),携带CC基因型的高血压合并房颤患者左心房内径明显增高(P〈0.05)。结论CYP11B2基因-344T/C多态性与原发性高血压患者房颤的发生相关。 Objective To investigate the association of aldosterone synthase(CYP11B2) gene --344T/C polymorphism with atrial fibrillation(AF) in hypertensive patients in Yubei of Henan province,China. Methods In this case-control study,a total of 803 patients who had essential hypertension were recruited. Of them, 405 had AF and 398 had normal sinus rhythm(Non-AF). CYP11B2 --344T/C polymorphism was determined by PCR-RFLP approach. Results There was no significant difference in allele frequency between the two groups (x2 = 1.32,P =0. 25). The frequencies of TT,CT,and CC were 37.0%,38.5% and 24.4%,and 34.2%,52.3% and 13.6% respectively, there was significant difference in the genotype distribution (x2 = 21.29, P 〈 0.05). Compared with T/T or T/C genotype,homozygosity for the C/C genotype significantly increased the risk of developing AF in patients with hypertension with the odds ratio being 1.96(95% CI: 1.23-2.67) and significantly increased the left atrial diameter in patients with AF and hypertension. Conclusion The main results in the present study suggest that the development of AF in patients with hypertension is associated with CYP11B2 genotypes, polymorphism of CYP11B2 --344T/C may play a role in occurrence of AF in hypertensive patients.
