摘要
目的：观察中西医结合治疗糖尿病肾病（ Ｄ Ｎ）的疗效。方法：６０ 例 Ｄ Ｎ 患者随机分为 ２ 组，中西组 ３０例采用中药方剂，每日１ 剂口服并配合西药达美康治疗；对照组单用达美康口服治疗。２ 组均３ 个月为 １ 个疗程。结果：中西组总有效率为８３３％ ，对照组为５３３％ ，２ 组比较有显著性差异（ Ｐ＜ ００５）。中西组治疗后血、尿β２ 微球蛋白（β２ Ｍ Ｇ）、尿微量白蛋白（ｍ Ａｌｂ）均较治疗前明显降低（ Ｐ＜ ００１， Ｐ＜ ００５ 和 Ｐ＜ ０００１），治疗后血β２ Ｍ Ｇ 和尿ｍ Ａｌｂ ２ 组间比较也有显著性差异（ Ｐ＜ ００５ 和 Ｐ＜ ００１），而 ２ 组治疗前后血肌酐和血尿素氮均无显著性差异（ Ｐ 均＞ ００５）。结论：中西医结合治疗后 Ｄ Ｎ 症状改善或消失，肾功能也有一定改善，在降低尿ｍ Ａｌｂ 和血β２ Ｍ Ｇ 方面明显优于单纯西药治疗。
Objective To observe the curative effects of integrated traditional Chinese medicine (TCM) and western medicine (WM) on diabetic nephropathy (DN).Methods:60 cases with DN were randomly divided into two groups.The treatment group of integrated TCM WM( n =30) was treated with decoction of TCM once a day combined with diamicron while the control group ( n=30 ) treated with diamicron alone.Three months were a therapeutic course for both two groups.Results:The total effective rate in treatment group was 83 3% but it was 53 3% in control group.In comparison between two groups the difference was significant ( P <0 05).After therapy in treatment group all the concentrations of blood and urinary β 2microglobulin ( β 2MG ) and urinary microalbuminuria(mAlb) were significantly lower than those before therapy ( P <0 01, P <0 05 and P <0 001), meanwhile blood β 2MG and urinary mAlb after therapy in comparison between two groups the differences were significant ( P <0 05 and P <0 01) but before and after therapy in two groups the differences in serum creatinine (SCr) and blood urea nitrogen(BUN) were not significant(both P >0 05). Conclusions: After treatment with integrated TCM WM for DN the symptoms are improved or disappeared,renal functions are also improved to a certain extent,and its effects on depressing urinary mAlb and blood β 2MG are significantly superior to those treated with WM alone.
