脂肪细胞脂肪酸结合蛋白与糖尿病肾病早期的相关性研究 被引量:4

摘要 目的探讨脂肪细胞脂肪酸结合蛋白(FABP4)与糖尿病肾病早期的相关性。方法实验分为糖尿病肾病早期组46例和正常对照组40例,用ELISA方法检测FABP4,检测空腹血糖(FBP)、糖化血红蛋白(HbA1c)、胰岛素、血脂和肌酐(Scr),根据公式计算胰岛素敏感指数(ISI)和肾小球滤过率(eG-FR),用SPSS13.0统计软件进行统计分析。结果①糖尿病肾病组的体重指数(BMI)、FBG、三酰甘油(TG)、胆固醇(TC)、Scr和FABP4均较对照组增加(P<0.05),而ISI和eGFR则明显降低(P<0.05);②Spearman相关分析显示糖尿病肾病组血浆FABP4与BMI、FBG、HbA1c、TG、Scr正相关,r分别为0.16、0.17、0.22、0.20、0.31(P<0.05)。而与ISI和eGFR负相关r分别为-0.32(P<0.01)、-0.44(P<03.01)。结论糖尿病肾病早期患者的FABP4水平升高,与肾功能具有负相关性。 Objective To evaluate whether plasma FABP4 is associated with renal dysfunction in the early stages of drabetre nephropathy.Methods 46 cases type 2 diabetic patients with microalbuminuria and 40 age-matched healthy nondiabetic controls were recruited.FABP4 levels were analyzed with enzyme-linked immunosorbent assay.Fasting blood glucose(FBG),fasting insulin,HbA1c,serum lipids and plasma creatinine were analyzed.The insulin sensitivity index(ISI)was applied to assess the status of insulin resistance,and we assessed renal dysfunction by estimated glomerular filtration rate(GFR)using the modification of diet in renal disease(MDRD)equation.Statistical analysis used SPSS 13.0 software.Results ①Compared with control subjects,BMI,FBG,TC,TG,Scr and FABP4 in type 2 diabetic patients with microalbuminuria were significantly elevated(P〈0.05),while ISI and eGFR were decreased(P〈0.05).②Spearman's correlation analysis showed that FABP4 was positively correlated to BMI,FBG,TG,HbA1c,TG,Scr.r:0.16,0.17,0.22,0.20,0.31(P〈0.05)respectively,and that it was negatively correlated to ISI and eGFR.r:-0.32(P〈0.01),-0.44(P〈0.05).Conclusion The level of FABP4 has increase in the early stages of diabetic nephropathy.It has the negative correlation with renal functiol.
