中老年人正位腰椎及髋部骨密度扫描对骨质疏松诊断敏感性的比较 被引量:14

Comparison of the sensitivity of osteoporosis diagnosis between using anterior-posterior lumbar spine scan and hip scan with DEXA in middle-aged and elder people

摘要 目的比较双能X线骨密度仪腰椎正位扫描及髋部扫描对骨质疏松诊断的敏感性。方法通过病史询问排除腰椎骨折的患者后,随机选取来我院骨密度室行骨密度检测的40至82岁中老年人340名(其中男性108名,女性232名)作为研究对象。采用双能X线骨密度仪对被检者腰部正位及髋部行骨密度检测,分析比较此两测量部位对骨质疏松症诊断的敏感性。结果男性被检者髋部骨密度扫描骨质疏松的检出率为62.03%高于腰椎正位的23.14%,结果有统计学差异,各年龄组髋部最低T分值均低于腰椎正位最低T分值,各组结果均有统计学差异。女性被检者髋部骨密度扫描骨质疏松的检出率为56.89%高于腰椎正位的50%,结果有统计学差异,各年龄组髋部最低T分值均低于腰椎正位最低T分值,各组结果均无统计学差异。结论 中老年人髋部T评分低于正位腰椎;髋部骨密度扫描较正位腰椎骨密度扫描对骨质疏松的诊断具有更高的敏感性;髋部T评分在骨质疏松诊断中的意义更为重要。 Objective To compare the diagnostic sensitivity of osteoporosis between anterior-posterior (AP) lumbar spine scan and hip scan using DEXA. Methods Three hundred and forty middle-aged and elder people (108 man and 240 women, 40-82 years old) were randomly selected for the study, excluding the patients with lumbar spine fracture by history taking. BMDs of the AP lumbar spine and the hip were measured using DEXA. The measurement sensitivity between the two sites was compared, Results Detection rate of osteoporosis with BMD scan was higher at the hip site (62.03%) than at the lumbar spine site (23.14%) in men (with statistic difference). The lowest T scores at the hip were lower than that at the AP lumbar spine in all age groups ( with statistic difference). Detection rate of osteoporosis with BMD scan was higher at the hip site (56. 89% ) than at the lumbar spine site (50%) in women (with statistic difference). The lowest T scores at the hip lower than that at the AP lumbar spine inall age groups (with no statistic difference). Conclusion The T score at the hip is low than that a the lumbar spine in middle-aged and elder people. Detection of osteoporosis at the hip site with BMD scan is more sensitive than that at AP lumbar spine. The T score of the hip plays more important role in the diagnosis of osteoporosis.
