摘要
目的探讨凋亡抑制蛋白P16在骨肉瘤中的表达及其与细胞凋亡的关系。方法采用免疫组织化学SP法检测52例骨肉瘤、15例骨软骨瘤及15例正常骨质标本的P16蛋白表达,运用TUNEL方法检测细胞凋亡。结果 P16蛋白在骨肉瘤中的表达显著低于骨软骨瘤和正常骨质(P<0.05),在肿瘤的组织学分级方面差别有显著性意义(P<0.05),而在性别、年龄、肿瘤直径、病理类型、侵袭及临床分期等方面差别无显著性(P>0.05)。凋亡指数(AI)在骨肉瘤中有无淋巴结转移、病理分型和组织学分级方面差别有显著性意义(P<0.05),有淋巴结转移组比无淋巴结转移组明显降低(P=0.048);纤维型和其他类型组比软骨母细胞型组明显降低(P=0.031);骨肉瘤Ⅱ级和Ⅲ级组比Ⅰ级组明显降低(P<0.05)。骨肉瘤中P16蛋白和AI表达之间存在正相关(r=0.425,P=0.002)。结论抑癌基因p16在骨肉瘤中随着组织学分级的降低,阳性表达率有降低趋势,P16蛋白的失活可能促进肿瘤的恶性进展过程,测定P16蛋白的表达和细胞凋亡可作为判断骨肉瘤恶性程度和预测预后的重要参考指标。
Objective To investigate the expression of the inhibitor of apoptosis protein P16 and cell apoptosis in osteosarcoma.Methods Expression of P16 protein was detected by immunohistochemistry staining(SP) assay in 52 osteosarcoma cases,15 osteochondroma cases and 15 normal bone cases.The apoptosis of osteosarcoma cell was assessed by terminal deoxynucleotide transferase mediated dUTP nick end labeling(TUNEL).Results The P16 protein expression was lower in the osteosarcoma than in control groups(P0.05).There was significant difference in pathological grade.No significant differences were observed in sex,year,size of tumor,types of pathology,invasion and clinical stages.Significant difference for average apoptosis index(AI) of osteosarcoma tissues was detected in tumor lymph node metastasis,pathological type and pathological grade(P0.05).Average AI in osteosarcoma tissues with lymph node metastasis group was significantly lower than that without lymph node metastasis group(P=0.048);Average AI in osteosarcoma tissues of fiber type and other type groups was significantly lower than that of chondroblastic group(P=0.031);Average AI of grade Ⅱ and Ⅲ osteosarcoma tissues groups was significantly lower than that of grade Ⅰ group(P0.05).The expression of P16 protein was significantly correlated with cell apoptosis in osteosarcoma(r=0.425,P=0.002).Conclusion The positive rate of expression of P16 protein tends to decrease with increasing pathological grade of osteosarcoma,which suggests that P16 protein may play an important role in the tumorigenesis and development of osteosarcoma.Detecting P16 protein and cell apoptosis may serve as a marker of predicting degree of maligmacy and prognosis of osteosarcoma.
