摘要
目的:比较欣普贝生与催产素用于足月妊娠促宫颈成熟及引产的效果。方法:将60例足月妊娠、无阴道分娩禁忌和前列腺素应用禁忌、单胎头位待产孕妇随机分为观察组30例和对照组30例。观察组孕妇阴道后穹窿放置欣普贝生1枚,对照组静脉微量泵泵入小剂量催产素。比较两组用药前后Bishop评分、引产成功率、剖宫产率和新生儿结局。结果:观察组用药6h、12h后Bishop评分分别为(5.41±0.86)分和(7.82±1.05)分,明显高于对照组,观察组促宫颈成熟效果优于对照组;观察组引产成功率为93.3%,对照组为53.3%,两组相比差异有统计学意义;观察组剖宫产率为23.3%,对照组为46.7%,两组相比差异也有统计学意义;两足新生儿结局比较无显著性差异。结论:欣普贝生用于足月妊娠孕妇促宫颈成熟及引产安全有效,可降低剖宫产率。
Objective:To compare the effects of dinoprostone and oxytocin on cervical ripening and induced abortion of full term pregnancy.Methods:60 full-term pregnant women(head position,single pregnancy,without contraindication of vaginal delivery and prostaglandin application) were divided into observation group(30 cases) and control group(30 cases);dinoprostone was put into the posterior fornix of pregnant women in observation group for one pellet,and small dose oxytocin was pumped into vein of pregnant women in control group;the Bishop scores,success rates of induced abortion,rates of cesarean section and neonatal outcomes in the two groups before and after treatment were compared.Results:At 6 and 12 hours after treatment,the Bishop scores in observation group were(5.41±0.86) and(7.82±1.05),respectively,which were significantly higher than those in control group,the effect of cervical ripening in observation group was superior to that in control group;the success rates of induced abortion in observation group and control group were 93.3% and 53.3%,respectively,there was significant difference between the two groups;the rates of cesarean section in observation group and control group were 23.3% and 46.7%,respectively,there was significant difference between the two groups;there was no significant difference in neonatal outcomes between the two groups.Conclusion:Dinoprostone is a safe and effective drug for cervical ripening and induced abortion,which can reduce the rate of cesarean section.
