动态监测血乳酸对危重症患儿预后评价的意义 被引量:6

摘要 目的 探讨血乳酸水平与小儿危重病评分(PCIS)之间的相关性及血乳酸动态监测指标与患儿预后的关系.方法 比较乳酸升高组及乳酸正常组的住院情况;按PCIS评分分组,探讨血乳酸值与PCIS评分的相关性;根据预后将乳酸升高的危重患儿分为死亡组和存活组,比较两组患儿的乳酸指标和反映脏器/系统功能的临床指标的差异,利用Logistic回归分析的方法找出与预后显著相关的指标.结果 血乳酸升高组患儿的PCIS评分显著降低,休克发生率、MODS发生率及住院病死率明显上升,与乳酸正常组相比差异有统计学意义;随着PCIS评分降低,入ICU乳酸值、乳酸峰值明显增高,与PCIS评分有显著的线性相关关系(r分别为-0.455、-0.504,P<0.01);高乳酸血症患儿中,乳酸峰值、12 h乳酸清除率与患儿的预后明显相关.结论 血乳酸水平与PCIS评分存在明显线性相关关系,乳酸峰值、12 h乳酸清除率是评价患儿预后的良好指标. Objective To study the correlation between blood lactic acid level and PCIS scroing system.To study the relationship between dynamic lactic acid monitoring indexes and prognosis of critical patients.Methods Differences of mortality,incidence of MODS and shock,PCIS score between two groups were compared.The patients were divided into 3 groups by PCIS score to compare pathogenetic conditions prognosis, then to study the were divided into death group and survival group by the hospital prognosis. Differences of their lactic indexes and other clinical indicators which reflected organ/system status were compared, prognostic-significant lactic indexes were selected by mu fti-variable logistic analysis. Results Patients with elevated lactic acid level had more incidence of shock and MODS, higher PCIS scores and higher mortality, the differences had statistical significance compared with the normal lactic acid group. There were significant linear correlation between ICU lactic acid level, peak lactic acid level and PCIS scores respectively. For the patients with elevated lactic acid level, peak lactic acid level, 12h lactic clearance had significant correlation with prognosis. Conclusion PCIS scoring system had a significant linear correlation with lactic acid level. For the patients with elevated lactic acid level,peak lactic acid level,12h lactic clearance were good indicators to evaluate patients' prognosis.
