摘要
目的：研究经皮穴位电刺激对硬膜外麻醉阻滞范围的影响。方法：观察两种麻醉方法对硬膜外阻滞范围的影响，共施行胆囊切除术120例，其中经皮穴位电刺激配合小剂量硬膜外麻醉胆囊切除组（A组）60例，单纯小剂量硬膜外麻醉胆囊切除组（B组）60例。硬膜外麻醉用药种类、浓度（1．5％利多卡因）、给药程序及辅助用药相同，观察两种术前、术中及术后硬膜外导管中水柱的高度来间接观察硬膜外腔的压力变化。结果：A组减少硬膜外麻醉用药量（1．5％利多卡因）34．66％（约1／3）；A、B两组硬膜外腔注入相同局麻药量（1．5％利多卡因9mL），A组较B组阻滞神经节段宽2～3个，A组水柱高度术中及术后平均较诱导前低3．37cmH2O、4．83cmH20，B组水柱高度术中及术后平均较术前高3．64cmH2O、3．19cmH2O。结论：经皮穴位电刺激配合硬膜外麻醉可使局麻药易于渗透或使硬膜外腔渗透压降低。
Objectve：To study the effect of transcutaneous electrical stimulation on the block scope by epidural anesthesia. Method： 120 cases were divided into two groups, A group and B group. 60 cases in A group were done by transcutaneous electrical stimulation combined with low dose epidural anesthesia for cholecystectomy. And the other 60 ones in B group were done by low dose epidural anesthesia also for cholecystectomy. The drug types of epidural anesthesia, concentration （1.5% lidocaine）, administration procedures and adjuvants were the same. Before and after surgery and during the operation, the epidural pressure variations were observed by observing the water column height in epidural catheter which was to indirectly judge the epidural pressure. Result： In A group epidural anesthesia dosage （1.5% lidocaine） was reduced 34.66% （about 1/3）. After injected with the same dosage of local anesthetic in cavitas epiduralis （ 1.5% lidocaine 9ml）, the blocked nerve segments in A group was wider 2 - 3 than that in B group. And the heights of water column in A group during the operation and after the operation were stepped down 3.37 cmH20 and 4. 83 cmH20, but the heights of water column in B group during the operation and after the operation were increased 3.64 cmH20 and 3.19 cmH20. Conclusion： Transcutaneous electrical stimulation combined with epidural anesthesia makes anesthetics easy to permeate or makes epidural osmotic pressure decrease in cavitas epiduralis.
