摘要
目的:探讨补肾健骨中药预防肾病综合征患者激素性股骨头坏死的效果。方法:将符合标准的60例肾病综合征患者随机分为2组,每组30例。治疗组口服补肾健骨中药胶囊,对照组给予安慰剂。①在给药期间出现明显股骨头坏死症状者行MRI检查,6个月给药期内未出现明显股骨头坏死症状者在给药结束后行MRI检查评估股骨头坏死情况。②评估给药前后患者肾虚证候总积分。③在实验开始后3个月、6个月时分别行血分析、尿分析、肝肾功能和心电图以评估药物安全性。结果:①6个月内治疗组无股骨头坏死发生,对照组3例6髋发生坏死,差异有统计学意义(P=0.027)。②给药6个月后治疗组和对照组肾虚证候积分均较治疗前降低(t=15.989,P=0.000;t=14.198,P=0.000),且治疗组证候积分降低值大于对照组(t=9.709,P=0.000)。③两组均未发生不良反应。结论:补肾健骨法对预防肾病综合征患者激素性股骨头坏死有一定的效果,值得多中心大样本的临床研究证实其疗效以及深入的基础研究探索其机理。
Objective：To investigate the preventive effect of Reinforcing Kidney therapy on steroid-induced femoral head necrosis in patients with nephritic syndrome.Methods：60 cases met the criterion of nephritic syndrome were randomly divided into 2 groups,30 cases in each group.Patients in treatment group received capsule full of reinforcing kindney herb for 6 months,while patients in control group received placebo.①When significant symptom of femoral head necrosis appeared or at the end of treatment,all patients underwent bilateral hip MRI to screen for necrosis of the femoral head.②The kidney-deficiency scores were evaluated and documented before and after the treatment.③Blood,urine,liver and kidney function,and EKG were analyzed at the third months and sixth months of treatment for drug safety evaluation.Results：①Within the 6 months,necrosis occurred in bilateral femoral heads of three patients in control group and none femoral head necrosis occurred in treatment group on MRI.There was significant difference between the 2 groups（P=0.027）.②The kidney-deficiency scores decreased in both groups after 6-month treatment（t=15.989,P=0.000;t=14.198,P=0.000）and the scores of treatment group decrease more（t=9.709,P=0.000）.③There were no adverse effects documented.Conclusion：Reinforcing Kindney therapy may has some preventive effect on femoral head necrosis in patients sustained nephritic syndrome with application of steroid.Multi-center clinical trial and basic research are needed for further study.
