摘要
目的:评价细胞DNA定量分析系统在宫颈癌及癌前病变的诊断及预测作用。方法:通过对8670名妇女妇科筛查,对其中351例结果异常的患者给予阴道镜检查和病理组织学检查,比较细胞DNA定量分析结果和组织学检查结果,计算细胞DNA定量分析结果对宫颈病变的阳性预测值。结果:TBS分级ASCUS以上、DNA定量分析可见或可见大量DNA倍体异常细胞。对CINⅡ/Ⅲ级以上病变的敏感性、特异性及预测值:ASCUS以上者70例,敏感性26.70%,特异性74.71%,阳性预测值27.38%;可见大量DNA异倍体细胞84例,敏感性81.05%,特异性18.42%,阳性预测值87.56%;细胞学结合DNA定量分析的敏感性、特异性及阳性预测值分别为92.45%、7.47%、93.05%。结论:细胞DNA定量分析方法对CINⅡ、CINⅢ以上的宫颈病变有较高的敏感性,有利于宫颈癌和癌前病变的早期诊断和预测。
Objective Appraise diagnostic and prediction value of quantitative analysis of cell DNA for cervical lesions.Methods A total of 8 670 women were involved in this study,and 351 cases of abnormal results were undergone colposcopy and positioning biopsy for histopathological examination.The results of DNA Ploid analysis were compared with histology results to calculate the positive predictive value of it.Results For people of CIN Ⅱ/Ⅲ or above,the sensitivity of 26.70% and specificity of 74.71% and positive predictive value of 27.38% were calculated by routine cytology examination,while the sensitivity of 81.05% and specificity of 18.42% and positive predictive value of 87.56% by quantitative DNA using automation scanning diagnosis,the sensitivity of 92.45% and specificity of 7.47% and positive predictive value of 93.05% were calculated by both of them.Conclusion There is a high sensitivity in screening cervical lesions by using quantitative analysis of cell DNA,it is helpful to the early prediction and diagnosis of cervical cancer and precancerous lesions.
