摘要
目的：对比观察口服大剂量醛氢叶酸、５ＦＵ、ＤＤＰ（ＰＦＬ）与ＰＦ方案联合治疗对晚期及复发转移的鼻咽癌的疗效。方法：１９９６～１９９８年间，１００例局部晚期及复发转移的鼻咽癌，非随机分为ＣＦ组（ＰＦＬ方案）５１例，对照组（ＰＦ方案）４９例。晚期初治采用放化疗综合治疗（Ｃ＋Ｒ），复发转移采用单纯化疗（Ｃ）。结果：ＣＦ组初治２８例，其ＣＲ率、有效率分别为６４２％、８５７％，中位生存期２１月（４～３３月）。对照组２９例，其ＣＲ率、有效率分别为５９８％、８３２％，中位生存期１９８月（３３１月）。两组间无显著性差异（Ｐ＞００５）。ＣＦ组复发转移２３例，其ＣＲ率、有效率分别为１７４％、６０８％，中位生存期１２月（３～２０月）。对照组复发转移２０例，其ＣＲ率、有效率分别为１０％、４５％，中位生存期１０月（１５～１８月）。两组ＣＲ率及有效率有显著性差异（Ｐ＜００５），中位生存期无显著性差异（Ｐ＞００５）。ＣＦ组口腔炎、恶心、呕吐、脱发更多见（Ｐ＜００５），血液毒性无显著性差异（Ｐ＞００５）。结论：放化疗综合治疗对局部晚期鼻咽癌治疗效果较好。ＰＦＬ方案为鼻咽癌较为有效的治疗方案。
Objective: To evaluate the activity and safety of combined chemotherapy
including highdose leucovorin (citrovorum factor , CF), 5FU, DDP(PFL) and
PF(DDP+5FU)regimen on locally advanced and recurrent NPC. Methods: From 19961998, 100
patients (pts) with locally advanced or recurrent NPC were divided into 2 groupsCF group
(PFL):51 cases,in which previously untreated NPC with locally advanced NPC received
radiochemotherapy(C+R):28 cases,recurrent NPC received only
chemotherapy(C):23cases.Controt group(PF):49 cases,in which C+R:29 cases and C:20 cases.
Results: CF group (C+R) :CR rate and effective rate(ER) were respectively 642%, 857%, median
survival duration(MSD)21 m(433m). Control group(C+R):CR rate and ER were respectively 598%,
832%, MSD 198m(331m). There was no significant difference between the two groups(P> 005).
CF group(C):CR rate and ER are respectively 174%, 608%, MSD:12 m(320m). Contrast
group(C):CR rate and ER are respectively 10%, 45%, MSD:10m(1518m). There was significant
difference between the two groups in CR rate and ER (P < 005), but no significant difference in
MSD(P> 005). There were more mucositis, nausea, vomiting, alopecia in CF group(P < 005).
There was no significant difference in bone marrow suppression(P> 005). Conclusion: These
data showed the combination of the radiochemotheraphy was favorable in the treatment of local
advanced NPC. HDCF/5FU plus DDP(PFL) appeared to be more effective regime for NPC than
5FU and DDP (PF)
