摘要
目的探讨细胞凋亡及其调控基因B细胞淋巴瘤/白血病2(bcl2)、bcl2相关X蛋白(bax)基因的表达与药物终止早孕的关系。方法对随机采用米非司酮负压吸引(15例,米非司酮组)、米非司酮并米索前列醇(米索)(14例,米非司酮米索组)、负压吸引(15例,对照组)终止早孕经碘化丙锭染色的蜕膜与绒毛组织,应用流式细胞定性定量分析技术进行DNA分析,采用免疫荧光标记对bcl2、bax基因蛋白进行分析。结果米非司酮组、米非司酮米索组及对照组蜕膜凋亡水平分别为(14.0±3.9)%、(22.9±6.3)%、(5.3±9.3)%,绒毛凋亡水平分别为(10.9±2.8)%、(13.9±4.7)%、(4.6±3.0)%,米非司酮组与米非司酮米索组蜕膜与绒毛组织细胞凋亡水平较对照组明显升高(P<0.01~0.001),DNA直方图上均有亚二倍体峰。蜕膜中,米非司酮米索组较米非司酮组细胞凋亡明显升高(P<0.01),两组的蜕膜细胞凋亡水平明显高于绒毛(P<0.01),蜕膜bcl2/bax比值分别为1.4±0.7、0.9±0.3和1.4±0.7,绒毛bcl2/bax比值分别为1.0±0.4、0.8±0.3和1.4±0?
Objective To study relatinship of decidual and villous cells apoptosis and its control gene Bcell lymphoma/leukemia2(bcl2), bcl2 associated X protein (bax) with medical abortion. Methods Forty four women who volunteered to terminate their pregnancy were divided into 3 group: mifepristone group (exp. I, n=15), mifepristone with misoprostol group (exp. , n=14) and control group (n=15). The cell cycles of DNA of the decidual and villous cells stained with Propidium Iodide (PI) were analyzed and bcl2 and bax gene protein labeled with immunofluorescence were detected by flow cytometry. Results The percentage of decidual cells apoptosis were (14.03.9)%, (22.96.3)%, (5.39.3)% and of villous apoptosis were (10.92.8)%, (13.94.7)%, (4.63.0)% in exp. , exp. and control group respectively. The apoptotic percentages in decidual and villous cells of exp. and exp. groups were significantly higher than that of control (P<0.010.001). On DNA histogram, apoptosis peaks appeared in all the groups. The decidual cell apoptosis of exp. groups was significantly higher than that of exp. group (P<0.01). Both decidual apoptotic percentages of exp. and exp. groups were significantly higher than that of villous (P<0.01). Decidual bcl2/bax ratio were 1.40.7, 0.90.3, 1.40.7 and villous bcl2/bax ratio were 1.00.4, 0.80.3, 1.40.6 in the 3 groups respectively. Both decidual and villous bcl2/bax ratio of exp. group was significantly lower than that of control (P<0.050.01) which was in consistent with apoptosis. Conclusions These data indicated that mifepristone with misocprostol for terminating human early pregnancy might be realized by inducing decidual and villous cells excessive apoptosis. The descent of bcl2/bax ratio could be one of the factors of inducing decidual and villous apoptosis.
