摘要
目的了解本院血液真菌感染的菌种分布和耐药情况,为临床用药提供依据。方法血培养采用BD9120全自动血培养仪,分离培养采用沙保弱培养基,鉴定采用科玛嘉念珠菌显色培养基及API20C酵母样真菌鉴定板,药敏试验采用ATB FUNGUS 3微量稀释板。结果 29 801份血培养共分离出真菌236株,分离率0.79%,其中白假丝酵母菌110株(46.61%),近平滑假丝酵母菌60株(25.42%),光滑假丝酵母菌35株(14.83%),热带假丝酵母菌12株(5.08%),其他(新生隐球菌、阿萨丝孢酵母、其他酵母菌、丝状真菌)19株(8.05%),未分离出克柔假丝酵母菌;光滑假丝酵母菌和阿萨丝孢酵母对氟康唑为剂量依赖敏感,假丝酵母菌属其他种对氟康唑等5种抗真菌药物MIC50、MIC90均在敏感范围。结论血液真菌感染分离株多数为假丝酵母菌属真菌,除光滑假丝酵母菌外该属真菌对氟康唑等常用抗真菌药物敏感。
Objective To study the distribution and drug resistance of fungi in blood and provide the evidence for clinical therapy.Methods The blood was cultured with BD9120.The fungi were isolated with the SDA and CHROM agar colored medium and identified with API20C.The susceptibility test was then performed with FUNGUS 3 micro dilution plate.Results Totally 236 fungi strains were isolated from 29 801 blood specimen,the isolation rate was 0.79%.The prevalence of C.albicans was 110(46.61%),C.parapsilosis 60(25.42%),C.glabrata 35(14.83%),C.tropicalis 12(5.08%),others(Cryptococcus,Trichosporon asahii,other yeast,filmentons fungus) 19(8.05%),no C.Krusei was isolated.Susceptible-dose dependent(S-DD) to fluconazol was found in C.glabrata and Trichosporon asahii,MIC50 and MIC90 of other candida species to 5 antifungal drugs as fluconazol were in susceptible range.Conclusion The majority of fungi isolated from blood were candida species,which were susceptible to antifungal drugs,such as fluconazol,except C.glabrata.
