摘要
目的通过比较袢电外科切除术(loop electrosurgical excision procedure,LEEP)和射频自凝刀治疗宫颈糜烂的临床疗效,评价LEEP在治疗宫颈糜烂方面的临床价值,以寻求治疗宫颈糜烂更好的方法。方法选取我院门诊就诊的宫颈糜烂患者200例,根据宫颈糜烂深浅分为单纯型、颗粒型和乳头型,分为2组(LEEP组和射频自凝刀组),每组各100例,分别采用LEEP和自凝刀进行治疗,观察其疗效及不良反应,比较2种方法在各型间的疗效差异。结果 2种方法治疗后LEEP组和自凝刀组总有效率分别为100%和98%,一次性治愈率分别为98%和92%,2种方法在单纯型和颗粒型中疗效差异无统计学意义,乳头型LEEP和自凝刀治疗总有效率分别为100%和83.3%,2者比较差异有统计学意义(P<0.05)。2组治疗后的不良反应大致相同,但是手术时间、术中出血量自凝刀组明显优于LEEP组。结论 LEEP和射频自凝刀是目前治疗宫颈糜烂非常有效的方法,对于单纯型、颗粒型宫颈糜烂,2种治疗方法效果均好,对于乳头型宫颈糜烂,选择LEEP更合适,LEEP可做为射频自凝刀无效时的补充治疗手段。
Objective To evaluate the clinical value of LEEP for cervical erosion by comparing the clinical curative effects of loop electrosurgical excision procedure (LEEP) and radio frequency electronic knife (RFEK) and to seek for a better way to curing cer- vical erosion. Methods 200 cases of cervical erosion had visited our hospital were selected for subject which fell into three types ( simplex type, granular type and papillar type) according to the degree of cervical erosion. The patients were randomly divided into two groups, LEEP group and RFEK group (100 cases for each group). The curative effect and adverse reaction of the two groups were observed and compared. Results Total effective rates of LEEP and RFEK groups were 100% and 98% respectively and one-time cure rates were 98% and 92% respectively. No significant differences were found in curative effects between simplex type and granular type, while there was significant difference (P 〈 0.05 )in the curative effects between LEEP( 100% ) and RFEK(83.3% ) for papillar type. The adverse reactions of the two groups were much the same, however, RFEK group was apparently better than LEEP group in surgery time and intra-operative blood volume. Conclusion LEEP and RFEK are both effective methods to cervical erosion, the clinical curative effect on simplex type and granular type are pretty good, while LEEP is better for papillar type, LEEP can be looked as a supplementary therapy method of RFEP.
