摘要
目的:通过对重症监护病房(ICU)耐甲氧西林金黄色葡萄球菌(MRSA)感染的分析,探讨合理使用抗生素及防治措施以预防医院内MRSA感染的发生和流行。方法:对我院ICU2008年7月1日至2009年10月30日近15个月的追踪观察,对102例MRSA引起的医院内感染进行回顾性临床耐药性分析。结果:我院ICU病房15个月内共发生MRSA感染102例,占金黄色葡萄球菌感染的96.2%,均全部使用过广谱抗生素,使用大于2种抗生素占41%(OR值1.46,95%CI 1.54～3.17),大于3种侵入性操作占56%(OR值4.80,95%CI 2.27～10.29),大于3种基础疾病占49%(OR值2.88,95%CI 1.40～6.02)。结论:ICU中MRSA感染率较普通病房高,必须引起重视,控制MRSA感染应积极进行病原学监测、及时发现病例、隔离和治疗患者、合理使用广谱抗生素、严格消毒隔离措施、认真洗手等。
Objective： To analyze the clinical features and prevalence of nosocomial methicillin-resistant staphylococcus aureus（MRSA） infection in the intensive care unit（ICU） and to explore reasonable use of antibiotics and control measures to prevent MRSA.Methods： Retrospective analysis of 102 cases with MRSA infection from July 1,2008 to Oct 30,2009 was carried out.Results： There were 102 cases of S.aureus infection in the ICU during the past 15 months,among which 92.5% were MRSA infection with broad-spectrum antibiotics.Forty-one percent of the cases treated with more than 2 antibiotics（OR value was 1.46,95% CI was 1.54-3.17）.Fifty-six percent of the patients received more than 3 invasive operations（OR value was 4.80,95%CI was 2.27-10.29）,and 49 % of the cases have been suffering from more than 3 basic diseases（OR value was 2.88,95% CI was 1.40-6.02）.Conclusion： The infection rate of MRSA in the ICU was higher than that in the public wards.To control MRSA infection,pathogenic monitoring should be used,and infected patients should be isolated,keeping the patients apart,rational utilization of antibiotics,medical facility sterilization strictly and washing hand are very important for MRSA infections control.
