摘要
【目的】探讨慢性阻塞性肺疾病(COPD)急性加重期凝血功能的变化及与血气分析相关性。【方法】选择本院COPD急性加重期患者75例(A组)、COPD稳定期患者50例(B组)和健康体检者30例(C组),分别检测三组的凝血四项:凝血酶原时间(PT)、活化部分凝血酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(Fbg)及D-二聚体(DD)、动脉氧分压(Pa02)及动脉二氧化碳分压(PaCO2),分析凝血指标与动脉血气值的相关性。【结果】A组患者Fbg和D-D含量显著高于B组患者和C组(P〈0.01),B组Fbg和D-D含量也显著高于C组(P〈0.01),PT、APTT、TT含量在三组间差异无统计学意义(P〉0.05);Fbg与PaO7呈显著负相关,与PaCO2呈显著正相关。【结论】急性加重期患者存在高凝状态,易并发血栓形成及弥散性血管内凝血,是导致本病死亡的主要原因之一,对COPD急性加重期患者行凝血功能和D-D水平的监测,有助于采取有效的防治措施,控制病情的发展;适当的抗凝治疗可改善疾病的预后。
[Objective]To explore the correlation between the changes of coagulation function and blood gas analysis in patients with chronic obstructive pulmonary disease(COPD) in acute exacerbation period. [Methods] A total of 75 patients with COPD in acute exacerbation period(group A), 50 patients with COPD in stable period(group B) and 30 healthy subjects(group C) in our hospital were chosen. Four coagulation parameters prothrombin time(PT), activated partial thromboplastin time (APTT), thrombin time(TT), fibrinogen (Fbg), D-dimer (DD), PaO2 and PaCO2 in three groups were measured. The correlation between coagulation indexes and arterial blood gas value was analyzed. [Results] Fbg and DD in group A were significantly higher than those in group B and C( P〈0.01), and those in group B were significantly higher than those in group C ( P 〈0.01). There were no significant differenees in PT, APTT and TT among 3 groups( P 〉0.05). Fbg was negatively correlated with PaO2, and significantly positively correlated with PaCO2. [Conclusion] Patients with COPD in acute exacerbation period have hypercoagulable state which is susceptibility to be complicated with thrombosis and disseminated intravaseular coagulation being one of the main causes of death. Monitoring the coagulation function and DD level may be helpful to take effective measures and control the disease pro gress. Proper antieoagulation therapy can improve the prognosis of the disease.
