摘要
目的比较腹腔镜与开腹手术治疗胃十二指肠溃疡穿孔的疗效。方法将50例胃十二指肠溃疡穿孔患者随机分为腹腔镜组和开腹组,每组各25例,观察和比较两组患者围术期和并发症发生情况。结果腹腔镜组患者的术中出血量、术后肠蠕动恢复时间、术后使用镇痛药次数和住院时间明显少于开腹组(均P<0.05);腹腔镜组患者手术时间和住院总费用比开腹组高,但差异无统计学意义(P>0.05)。腹腔镜组并发症发生比开腹组少,差异有统计学意义(P<0.05)。两组患者随访期间均未发生穿孔复发。结论腹腔镜胃十二指肠溃疡穿孔修补术安全有效,一定程度上优于传统开腹手术。
Objective To compare the efficacy of laparoscopy and open surgery for gastroduodenal ulcer perforation.Methods 80 patients with gastroduodenal ulcer perforation were randomly divided into the laparoscopic group（n=40）and open surgery group（n=40）.The perioperative states,complication incidence were recorded and compared between two groups.Results The intraoperative blood loss,postoperative time of the recovery of intestinal peristalsis and hospital stay were significantly less in laparoscopy group than in the open surgery group（all P0.05）,the operative time and total expense of hospital stay of laparoscopy group were slightly higher that those of open surgery group,but without significant difference（P0.05）;the cases of occur of postoperative complication was significantly less in laparoscopy group than in the open surgery group（P0.05）.No relapse of perforation happened in two groups during the follow-up.Conclusion Compared with the open surgery,the laparoscopic repair for gastroduodenal ulcer perforation is more effective and safer.
