摘要
目的探讨新生儿缺氧缺血性脑病(HIE)血脂氧素A4(LXA4)和内皮素(ET)-1的表达及其意义。方法选取47例HIE新生儿,分为轻度HIE组16例、中度HIE组16例和重度HIE组15例;同时选取同时期15例健康新生儿为对照组。分别于出生时、出生后第3天、出生后第7天检测血清LXA4及ET-1水平的变化。结果对照组、轻度HIE组及中度HIE组出生后第3天及出生后第7天LXA4水平较出生时均明显升高,差异有统计学意义(P<0.05)。重度HIE组出生后第3天LXA4水平较出生时明显降低,出生后第7天LXA4水平较出生后第3天明显升高,差异均有统计学意义(P<0.05)。出生时轻度、中度及重度HIE组患儿LXA4水平较对照组均明显升高,差异有统计学意义(P<0.05);出生后第3天轻度及中度HIE组患儿LXA4水平较对照组均明显升高,差异有统计学意义(P<0.05);出生后第7天轻度、中度、重度HIE组患儿LXA4水平较对照组均明显升高,差异有统计学意义(P<0.05)。轻度、中度及重度HIE组患儿出生后第3天及出生后第7天ET-1水平较出生时均明显降低,差异有统计学意义(P<0.05)。出生时轻度、中度及重度HIE组患儿ET-1水平较对照组均明显升高,差异有统计学意义(P<0.05);出生后第3天及出生后第7天中度及重度HIE组患儿ET-1水平较对照组均明显升高,差异有统计学意义(P<0.05)。结论重度HIE患儿合成LXA4的能力显著受损,LXA4在HIE发病过程中起着明显抗感染作用,出生后即刻HIE新生儿血ET-1水平升高,病情越重升高越显著。
Objective To detect the expression of LXA4 and ET-1 in HIE neonates and its clinical significance.Methods Forty-seven HIE neonates were divided into groups mild(n=16),moderate(n=16),severe(n=15),and 15 healthy neonates enrolled simultaneously as control group.The changes of serum LXA4 and ET-1 levels were determined instantly,or on days 3,7 after birth.Results LXA4 level was significantly higher on days 3,7 after than at birth in groups control,mild,moderate(P〈0.05);lower on days 3 than at birth in severe group(P〈0.05);higher on days 7 than on days 3 in severe group(P〈0.05);higher in groups mild,moderate and severe than in control at birth(P〈0.05);higher in groups mild,moderate than in control on days 3 after birth(P〈0.05);and higher in groups mild,moderate,severe than in control on days 7(P〈0.05).ET-1 level was significantly lower on days 3,7 than at birth in groups mild,moderate,severe(P〈0.05);higher in groups mild,moderate,severe than in control at birth(P〈0.05),and higher in groups moderate,severe than in control on days 3,7(P〈0.05).Conclusion In severe HIE neonates,the LXA4 synthesis ability is impaired obviously.LXA4 play a remarkable role in anti-inflammation.ET-1 level increases markedly in HIE neonates instantly after birth,and more markedly with severity of patients′ situations.
