摘要
目的观察胰岛素强化降糖对原发性开角型青光眼(POAG)合并2型糖尿病(T2DM)患者视野的保护作用。方法将66例眼压控制稳定的伴T2DM的POAG患者随机分为胰岛素强化治疗组(观察组,33例,46眼)和常规治疗组(对照组,33例,50眼)。观察组给予胰岛素强化治疗,对照组给予口服降糖药治疗,均治疗12周。观察治疗前后两组患者空腹血糖(FBG)、糖化血红蛋白(HbA1c)及视野的变化。结果 (1)治疗前观察组及对照组患者的FBG〔分别为(8.0±2.2)mmol/L、(8.1±2.8)mmol/L〕、HbA1c〔分别为(7.6±1.2)%、(7.7±1.2)%〕、视野平均缺损(MD)〔分别为(12.0±7.1)dB、(11.8±6.2)dB〕、平均光敏感度(MS)〔分别为(15.1±7.5)dB、(15.2±8.1)dB〕比较,差异均无统计学意义(P>0.05)。(2)治疗后观察组FBG〔(6.1±1.8)mmol/L〕、HbA1c〔(6.3±0.6)%〕、MD〔(10.5±6.2)dB〕均较对照组〔分别为(7.4±1.6)mmol/L、(7.1±1.3)%、(12.0±7.2)dB〕显著降低,MS〔(17.1±6.1)dB〕较对照组〔(14.8±7.9)dB〕显著升高,差异均有统计学意义(P<0.05)。结论胰岛素强化降糖可改善POAG合并T2DM患者的视野损害,保护视功能。
Objective To evaluate the protective effect of intensive blood glucose control on the visual field of primary open-angle glaucoma(POAG) patients with type 2 diabetes mellitus(T2DM).Methods Sixty-six POAG patients with T2DM under controlling intraocular pressure were divided into two groups: intensive treatment group(33 cases,46 eyes) and conventional treatment group(33 cases,50 eyes).Intensive treatment group were treated with intensive insulin while conventional group with oral antidiabetic drugs for 12 weeks.Analyze the changes of fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c) and visual field before and after treatment.Results(1)There were no significant differences in FBG((8.0±2.2)mmol/L vs(8.1±2.8)mmol/L),HbA1c((7.6±1.2)% vs(7.7±1.2)%),mean defect(MD)((12.0±7.1)dB vs(11.8±6.2)dB) and mean sensitivity(MS)((15.1±7.5)dB vs(15.2±8.1)dB) between two groups before treatment(P〉0.05).(2)After treatment,FBG((6.1±1.8)mmol/L),HbA1c((6.3±0.6)%),MD((10.5±6.2)dB) were significantly reduced in intensive group compared with conventional group((7.4±1.6)mmol/L,(7.1±1.3)%,(12.0±7.2)dB,P〈0.05);and MS((17.1±6.1)dB) was significantly increased in intensive group compared with conventional group((14.8±7.9)dB,P〈0.05).Conclusion Intensive blood glucose control can protect the visual field of POAG patients with T2DM.
