摘要
目的探讨64排螺旋CT及后处理技术在诊断单侧肺动脉异常起源的应用价值。方法收集经手术或造影证实的16例单侧肺动脉异常起源的患儿资料,对64排螺旋CT及后处理图像进行回顾性分析。结果左肺动脉起自右肺动脉6例;单侧肺动脉起自主动脉10例,其中右肺动脉起源于升主动脉5例,左肺动脉起源于升主动脉2例,右肺动脉完全缺如血供起自降主动脉侧枝2例,左肺动脉完全缺如血供起自降主动脉侧枝1例。并发其他心血管畸形36处,气道狭窄、肺、胸廓等畸形13处。超声心动图诊断单侧肺动脉异常起源12例,并发的心血管畸形33处,纵膈疝1处。64排螺旋CT和超声心电图诊断肺动脉异常起源的准确性分别是100%(16/16)和75%(12/16),诊断并发心血管畸形数的正确率分别为94.87%(37/39)和84.61%(33/39),差异均有统计学意义(P<0.05)。结论 64排螺旋CT及后处理技术诊断单侧肺动脉异常起源准确性高,对伴发的心血管畸形,尤其是心外血管畸形和气管、肺、胸廓等情况能很好地显示,是一种可靠的无创性检查方法。
Objective To investigate the diagnostic values of 64-slice spiral CT and post-processing technology in anomalous origin of unilateral pulmonary artery.Methods The data of 64-slice spiral CT results and processed results of 16 children(at a mean age of 18 months,ranging from 1 month to 6 years) with surgery or angiocardiography identified anomalous origin of unilateral pulmonary artery who admitted in our hospital from November 2008 to August 2010 were retrospectively reviewed.Results Left pulmonary artery anomalous origin from the right pulmonary artery was found in 6 cases,unilateral pulmonary artery from the aorta in 10 cases among which the right pulmonary artery anomalous origin in the ascending aorta in 5 cases,right pulmonary artery anomalous origin in the ascending aorta in 2 cases,right pulmonary artery absence completely and the blood supply of collateral from the descending aorta in 2 cases,and left pulmonary artery absence completely and the blood supply of collateral from the descending aorta in 1 case.In these 16 children,there were 36 cardiovascular deformities accompanying,and 13 other deformities,such as airway stenosis,lung,and thoracic deformities.Echocardiography diagnosed that there were 12 cases of anomalous origin of unilateral pulmonary artery,accompanied with 33 cardiovascular deformities,and diaphragmatic hernia in 1 case.Compared with the results of surgery or angiocardiography,the accuracy of 64-slice spiral CT and ultrasonography was 100%(16/16) and 75%(12/16) respectively in evaluating the anomalous origin of unilateral pulmonary artery,and was 94.87% and 84.61% in diagnosis of the number of cardiovascular malformations.There was statistically significant between the 2 diagnostic methods(P0.05).Conclusion 64-slice spiral CT have high accuracy in diagnosis of anomalous origin of unilateral pulmonary artery in children,and display cardiovascular malformations,particularly vascular deformities and the trachea,lung,thorax and other circumstances well.It is a good noninvasive diagnostic tool.
