摘要
目的探讨以不同置换液剂量行连续性静脉-静脉血液滤过(CVVH)联合血液灌流(HP)对蜂蛰伤并发急性肾损伤(AKI)疗效的影响。方法回顾性分析西南医院、新桥医院和解放军第452医院2003年1月至2009年11月行CVVH联合HP治疗的蜂蛰伤并发AKI患者45例的临床资料,按置换液剂量分为中小剂量组[35～45 mL/(kg.h)]、大剂量组[45～70 mL/(kg.h)]。结果两组患者年龄、体重指数、死亡率及血液净化治疗前肌酐、急性生理学和慢性健康状况评分Ⅱ(APACHEⅡ评分)无统计学差异(P>0.05);中小剂量组少尿期持续时间、平均连续血液滤过治疗时间明显长于大剂量组(P<0.05);两组经CVVH联合HP治疗24 h的APACHEⅡ评分无统计学差异(P>0.05),中小剂量组经CVVH联合HP治疗48 h的APACHEⅡ评分明显高于大剂量组(P<0.05)。结论以大剂量行CVVH联合HP有助于缓解蜂蛰伤并发AKI的病情。
Objective To investigate the therapeutic effect of wasp sting complicated with acute kidney injury（AKI） using different doses in continuous veno-venous hemofiltration（CVVH） combined with hemoperfusion（HP）.Methods We retrospectively studied 45 patients with wasp sting complicated with AKI from Southwest Hospital,Xinqiao Hospital and 452 Hospital of PLA,these patients were treated with CVVH combined with HP in the period of Jan.2003 to Nov.2009.According to the volume of substitute fluid in CVVH,all of them were divided into 2 groups： middle and lower dosage group[35-45 mL/（kg.h） ] and higher dosage group[ 45-70 mL/（kg.h） ].Results（1）Age,BMI,mortality rate in hospital,serum creatinine and APACHE Ⅱ score prior to CVVH combined with HP had no statistical deference be-tween 2 groups（P 0.05）.（2） Length of oliguria stage and length of CVVH in middle and lower dosage group were longer than those in higher dosage group（P 0.05）.（3） The APACHE Ⅱ score after CVVH combined with HP in 24h had no statistical deference between 2 groups（P 0.05）,the APACHE Ⅱ score in middle and lower dosage group after CVVH combined with HP in 48h was higher than that in higher dosage group（P 0.05）.Conclusion Higher dosage of substitute fluid in CVVH combined with HP is helpful to alleviate the disease condition of wasp sting complicated with AKI.
