摘要
目的评价微型钢板螺钉治疗Mason Ⅲ型桡骨小头骨折的临床治疗效果。方法 2002年1月至2010年3月共收治35例Mason Ⅲ型粉碎性桡骨小头骨折,均采用切开复位、掌指骨微型钢板螺钉内固定治疗,根据随访对患者的肘、腕关节的疼痛、运动、肌力、X线表现,以及肘关节功能评分进行分析研究。结果 35例患者随访6~14个月,平均12个月,Morrey肘关节功能评分平均85.3分,优良率为94.3%;VAS评分平均21.7分,肘携带角均无明显变化,无一例发生腕关节炎。结论切开复位、微型钢板螺钉内固定治疗Mason Ⅲ型粉碎性桡骨小头骨折可获得较满意的关节活动范围,得到较大的肌力恢复以及较好的关节功能恢复。
Objective To study the effect of miniplate and screw on Mason type-Ⅲ comminuted fractures of radial head. Methods 35 cases of Mason type-Ⅲ radial head fracture were treated from January, 2002 to March, 2010. All cases were treated by miniplate and screw after open reduction. The overall outcome were assessed based on of the pain, motion, strength, radiographic findings and the function rating score described by Broberg and Morrey. Results 35 cases had a mean follow-up of 12 months (6 to 14 months), the average pain VAS score was 21.7, and the mean Morrey score was 85.3. 94.3% patients got satisfactory results, there were no evident change in the elbow carrying angle and no wrist arthritis cases. Conclusion Open reduction and miniplate and screw internal fixation can result in more satisfactory elbow motion, greater muscle strength and better functional recovery in the treatment of Mason type-Ⅲ comminuted fractures of radial head.
