放射性^(125)Ⅰ粒子治疗肺恶性肿瘤疗效分析 被引量:2

摘要 目的评价放射性^(125)Ⅰ粒子组织间植入治疗肺癌的技术可行性和近期疗效。方法 2004年10月~2007年3月,82例因各种原因不能手术,或术中不能切除的肺癌患者纳入分析,分为常规放化疗组、碘^(125)Ⅰ粒子综合治疗组、单纯碘^(125)Ⅰ粒子植入组。利用三维治疗计划系统(TPS)精确制定等剂量曲线和制定计划,治疗的肿瘤全部靶区位于90%等剂量曲线之内。在CT或术中根据治疗计划进行肿瘤穿刺,将^(125)Ⅰ粒子植入到预定位置。比较术后患者肿瘤大小、生活质量,以及不良反应。结果 ^(125)Ⅰ粒子综合治疗组术后1个月比较肿瘤大小,有效率(CR+PR)88.09%。生存质量、不良反应有明显变化(P<0.05),中位生存率和中位生存时间、生存质量明显好于常规放化疗组(P<0.05)。单纯碘^(125)Ⅰ粒子置入组肿瘤大小明显好于常规放化疗组(P<0.05)。生存率优于常规放化疗组,处于统计学边沿水平。结论放射性粒子植入治疗肺恶性肿瘤具有创伤小,不良反应少,安全性高,生活质量高,疗效显著的特点。 Objective To explore the technical feasibility and short term effect of interstitial implantation of ^(125)Ⅰseeds for lung cancer.Methods From October,2004 to March,2007,82 patients with lung cancer who either couldn't take operation or couldn't remove tumor through operation were selected for this study and grouped into routine treatment group,Comprehensive treatment group and Pure implantation of ^(125)Ⅰseeds group.Using treatment planning system(TPS) accurate formulate isodose curve and formulate plans,treatment of tumor target zone is located in 90% of all such dose curve.The TPS guided through CT was used to establish 3 dimensional digital image and accurately calculate the number of seeds and the space distribution.In operation,^(125)Ⅰseeds were implanted into scheduled point.After operation,analysis was carried out over patients tumor size,quality of life,and side effect.Results One month following the operation,study of tumor size,effectiveness rate,life quality,side effect,tumor indicator all show improvement in comparison to the same before operation.Effective ratio of ^(125)Ⅰseeds treatment proves 88.09%.The median survival ratio,length of survival time and life quality are much better than the comparison group.Tumor size in the group with interstitial implantation of ^(125)Ⅰseeds alone.Is apparently better than comparison group.There is no difference in survival rate.But the difference is statistically insignificant.Conclusion CT-guided interstitial implantation of ^(125)Ⅰseeds is a safe and the least invasive method for lung cancer treatment with a low rate of complications and good outcomes.
