摘要
目的:探讨色素沉着绒毛结节性滑膜炎(PVNS)MRI表现及其病理基础。方法:搜集经关节镜及手术病理证实的18例PVNS患者资料,分析其MRI征象并与关节镜、病理结果对照。结果:18例均可见增生肥厚的滑膜和含铁血黄素沉着形成的结节(其中13例呈弥漫型,5例呈局限型):T1WI及FFE滑膜增厚呈等或稍低信号,T2WI呈稍高信号;含铁血黄素沉着结节呈长T1、短T2信号,FFE呈低信号。MRI还可显示不同程度的关节积液(18例)、关节软骨破坏(9例)及软骨下骨质内小囊状破坏(3例)、骨髓水肿(12例)、前交叉韧带损伤(1例)、半月板Ⅱ~Ⅲ度损伤(5例)。结论:MRI能够提供比较全面的影像学信息,可提高对PVNS诊断的准确性。
Purpose:To study the MRI manifestations and pathologic basis of pigmented villonodular synovitis(PVNS).Methods:Arthroscopy and surgical pathology confirmed 18 cases of PVNS patients were included in our study.The arthroscopic findings,MRI features,pathological findings were analyzed.Results:Proliferation of synovial hypertrophy and formation of nodules with hemosiderin deposition were seen in 18 cases(of which 13 cases were diffuse type,5 cases were localized to type).Thickened synovial membrane was shown on T1WI and FFE with low signal,on T2WI with slightly higher signal,hemosiderosis nodules were shown long T1 short T2 signals,low signal on FFE.Different degrees of joint effusion(18 cases),articular cartilage damage(9 cases),and small cystic of subchondral bone damage (3 cases),bone marrow edema(12 cases),anterior cruciate ligament injuries(1 cases),Ⅱ~Ⅲ degree of meniscus injury(5 cases)were also shown on MRI.Conclusion: MRI can provide more comprehensive information and can also improve the accuracy of diagnosis of PVNS.
