经颅磁刺激运动诱发电位对中风的临床评估

摘要 目的:本研究主要用于探讨MEP对中风患者临床体征及预后的评估价值。方法:收集18例急性中风患者于1周内行临床体格及MEP检查,2~3月后再随访一次.临床体检主要包括肌力检查.MEP由双侧小鱼际肌及胫前肌记录,将临床体检与MEP相联系、结果:本组研究8例MEP无反应者偏瘫较重.预后较差,而10例MEP有反应者偏瘫较轻,预后较好.两组差异有显著性。结论:在中风急性期,经颅磁刺激运动皮层是一个评估患者运动功能及预后的安全有价值的非侵袭性方法,是对临床及影象学技术的一个补充。 Objective:This tudy is to evaluate MEP in predicting stroke patient's clinical physicap cx-amination and prognosis. Methods: Eighteen acutc strokc patients were evaluated clinically and MEP with-in the first week, and 2 to 3 months were followcd up survcy. Clinical physicap examination included mus-cle strength. MEP were recorded from small thenar or tibialis anterior muscles bilaterally, Correlated clini- cal findings and MEPs. Results:In this study, no MEP could be detected in 8 patients whose hemiplegiawas severc and outcome was poor. while 10 patients whose MEPs could be elicitcd, hemiplegia was mildand outcome was good. Significant differences were obscrvcd between two groups. Conclusion: Transcra-nial magnctic stimulation of motor cortex during the acutc strokc provides a safe, valuable, non-invasivemethod for predicting motor function and prognosis, which complements clinical assessments and imagingtechniques.
