2型糖尿病患者餐后血脂与颈动脉内-中膜厚度的关系研究 被引量:7

The Relation between Postprandial Lipid Changes and Carotid Intima-media Thickness Value in Type 2 Diabetes Mellitus

摘要 目的探讨2型糖尿病(T2DM)患者的餐后血脂与颈动脉内-中膜厚度(CAIMT)的关系,明确T2DM早期发生动脉粥样硬化的机制。方法选取49例T2DM患者(DM组)及43例健康者(对照组),均进食脂负荷餐,采集空腹及餐后4 h的血标本,测定血糖、胰岛素及血脂〔三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)〕。使用彩色多普勒测量两组受检者的CAIMT。结果 (1)DM组患者的餐后4 h血糖、TG水平较对照组显著升高,餐后4 h HDL-C水平较对照组显著下降,差异有统计学意义(P<0.01);DM组患者CAIMT为(0.86±0.17)mm,显著高于对照组的(0.69±0.12)mm,差异有统计学意义(P<0.01)。(2)在DM组,对CAIMT(Y)进行逐步回归分析,回归方程为:Y=0.247+0.04X1+0.009X2+0.006X3,X1=餐后4 h TG,X2=空腹胰岛素,X3=年龄,决定系数(R2)=0.742,P<0.01。结论 T2DM患者较正常人有明显的餐后血脂代谢紊乱,且与早期动脉粥样硬化的发生相关。 Objective To study the relationship between the postprandial lipid changes and carotid intima -media thickness value in type 2 diabetes mellitus, so as to investigate the mechanism of early atheroselerosis in type 2 diabetes mellitus. Methods 49 patients with type 2 diabetes mellitus and 43 control subjects were recruited. Plasma glucose, triglyceride, total cholesterol, high density lipoprotein -cholesterol and low density lipoprotein -cholesterol were measured at fasting and 4 h after eating fat - load meal. Carotid intima - media thickness was measured by color Doppler apparatus in both groups. Results ( 1 ) The 4 h postprandial blood glucose and TG were significantly higher in DM group than in the control group, while the 4 h postprandial HDL - C was significantly lower in DM group (P 〈0. 01 ). The level of CAIMT in DM group (0. 86 ±0. 17) mm was significantly higher than that in coutrol group (0. 69 ±0. 12) mm (P 〈0.01 ). (2) CAIMT in DM group was analyzed by multivariate stepwise regression, the linear regression equation is: Y =0. 247 +0. 04X1 +0. 009X2 +0. 006X3, with X1 being 4 h postprandial TG, X2 being fasting insulin and X3 the age, R2 = 0. 742, P 〈0. 01. Conclusion Type 2 diabetes mellitus patients have an obvious postprandial dyslipidemia, especially postprandial hyperglyceridemia, which has significant correlation with atherosclerosis.
