摘要
目的探讨增强型体外反搏(EECP)治疗对高血压并糖耐量减低(IGT)患者肱踝脉搏波传导速度(b8PWV)及血管内皮功能的影响。方法38例高血压并IGT患者分为常规治疗组(CT,20例)和常规治疗+EECP组(CT+EECP,18例)。CT+EECP组隔天反搏治疗2h,总共反搏36h,共36d。分别于体外反搏前和反搏后检测baPWV,同时检测血浆内皮素(ET)、一氧化氮(NO).结果CT组和CT+EECP组治疗后baPWV明显下降,分别为1703±213(P〈0.05)和1601±208(P〈0.01),两组比较差异有统计学意义(p〈0.05).两组患者经治疗后ET均明显下降(74.8±21.3和62.6±20.8,P〈0.05,P〈0.01),NO均明显上升(67.7:1:17.4和78.2±18.2,P〈0.05,P〈0.01).CT+EECP组baPWV和ET下降更为显著,下降幅度与常规治疗组比较差异有统计学意义(P〈0.05)。结论增强型体外反搏可改善高血压并IGT患者血管内皮功能,降低血管僵硬度,从而改善动脉弹性。
Objective To explore the effect of Enhanced External Counterpulsation (EECP) on pulse wave velocity (baPWV) and vascular endothelial function in hypertensive patients complicated by impaired glucose tolerance (IGT). Methods Thirty-eight hypertensive patients complicated by IGT were randomly divided into 2 groups : the conventional therapeutics group ( CT, n =20 ), the conventional therapeutics +EECP group ( CT+EECP, n=18). EECP was performed on CT+EECP group, 2 hours every other day lasting for 36 days. baPWV, Plasma ET, Nitric oxide(NO) were measured at the same time points in all patients. Results In CT+EECP group, baPWV was decreased significantly (1601±208, P〈0.01 ) after treatment comparing to the CT group (1703±213, P〈 0.05). Level of endothelin(ET) was significantly decreased in CT+EECP group (62.6±20.8, P〈0.01 ), comparing to the CT group[ (74.8±21.3)mg/L, P〈0.05 ] after 36-day treatment. However, level of Nitric oxide(NO) was significantly improved in CT+EECP group (78.2±18.2, P〈0.01) comparing to the CT group (67.7±17.4)mg/L, P〈 0.05) after treatment. Conclusion EECP could improve endothelial function and artery elasticity in hypertensive patients complicated by IGT.
