TSEP和BAEP监测在三叉神经痛微血管减压术中的应用价值 被引量:4

The value of TSEP and BAEP monitoring during microvascular decompression in patients with trigeminal neuralgia

摘要 目的:探讨三叉神经痛微血管减压术(MVD)中联合应用三叉神经诱发电位(TSEP)、脑干听觉诱发电位(BAEP)监测的价值。方法:2007年2月至2009年11月我院采用MVD治疗58例三叉神经痛病人,以术中使用电生理监测的30例患者为试验组,未使用这两种电生理监测的28例患者为对照组,对比分析两组有效率(三叉神经痛症状缓解或消失)及眩晕、听力下降等并发症的发生率。结果:试验组术后有效率为93%(28例),发生眩晕1例(3%)和听力下降1例(3%);对照组术后有效率为68%(19例),发生眩晕8例(29%)和听力下降9例(32%)。两组MVD治疗疗效差异及并发症发生率比较差异均有统计学意义(P〈0.05)。结论:三叉神经痛MVD中,联合应用TSEP和BAEP作术中监测能较好地控制并发症的发生及提高三叉神经MVD的治疗效果。 Objective:To explore the value of trigeminal somatosensory evoked potential(TSEP)and brainstem auditory evoked potential(BAEP)monitoring during microvascular decompression in patients with trigeminal neuralgia.Methods..From Feb,2007 to Nov,2009,TSEP and BAEP were ferformed in 58 patients with trigeminal neuralgia during microvascular decompression.Thirty patients with intraoperative TSEP and BAEP monitoring were classified as the experimental group and the control group with 28 patients without intraoperative TSEP and BAEP monitoring.The surgical success rate and complications such as hearing impairment,vertigo of the two groups were statistically compared.Results:After surger y,the cure rate of operation was 92%(28 cases)and 68%(19 cases)in the experimental group and the control group respectively.In the experimental groups,vertigo occurred in 1 case(3%),hearing impairment in 1 case(3%).In the control group,vertigo in 8 cases(29%),and hearing impairment in 9 cases(32%):The surgical cure rate and complication in the experimental group and the control group respectively,there were statistical difference(P〈0.05).Conclusion:During microvascufar decompression for the patients with trigeminal neuralgia,intraoperative TSEP and BAEP monitoring are effective for avoidance of complications and can raise efficacy of trigeminal decompression.
