摘要
为探讨畸变产物耳声发射(DPOAEs)对噪声性聋的诊断价值,对46名长期高强度噪声暴露人群(72耳,其中噪声性聋52耳,听阈正常20耳)和48名(86耳)听力正常人在4组不同初始刺激强度下进行DPOAEs测试。结果:在L1=L2=60dBSPL初始刺激强度下,各频率DPOAEs反应幅值与对应纯音听阈相关系数最大,且长期高强度噪声暴露人群中的听阈正常耳与听力正常耳(对照组)的DPOAEs反应幅值相比无显著性差异,而噪声性聋患者与听力正常人(对照组)DPOAEs幅值相比有显著性差异(P<0.01)。结论:①DPOAEs检查可以客观地早期诊断噪声性聋;②L1=L2=60dBSPL为噪声性聋DPOAEs测试最适初始刺激参数。
In order to study the diagnostic value of distortion product otoacoustic emissions(DPOAEs) in the diagnosis of noiseinduced deafness, DPOAEs at the 2f1f2 frequency were recorded from 46 chronic highlevel noise exposure people (72 ears, including 52 ears suffered from noiseinduced deafness and 20 ears of normally hearing threshold) and 48 normally hearing subjects(control group,86 ears) at 4 different primary stimulus levels. The pure tone threshold of noiseinduced deafness ears at each frequency was used to make correlation analysis with the DPOAEs The results showed that the correlation coefficient at each frequency was the largest under L1=L2=60 dB SPL primary stimulus level Moreover, under the L1=L2=60 dB SPL primary stimulus, there existed no significant difference in DPOAEs response amplitude between normally hearing threshold ears from chronic highlevel noise exposure people and control group, but significant difference in DPOAEs amplitude between people suffered from noiseinduced hearing loss and control group(P<001) It was concluded that DPOAEs permit an unbiased means of diagnosing earlier stage of noiseinduced deafness; L1=L2=60 dB SPL is the best one at 4 different primary stimulus levels
