左右侧卵泡液中LIF、等含量与卵子质量的关系

摘要 目的 探讨左/右侧卵巢卵泡液中白血病抑制因子及雌二醇、孕激素、睾酮含量与体外受精-胚胎移植/卵胞浆内单精子注射卵子数量及质量的关系.方法 选择首次接受体外受精/卵胞浆内单精子注射周期患者37例,采卵日收集左/右侧卵巢卵泡液,用酶联免疫吸附法测定白血病抑制因子含量;用化学发光法测定雌二醇、孕激素、睾酮浓度.结果 ①右侧卵泡液中白血病抑制因子、雌二醇、孕激素含量明显高于左侧(t值分别为-2.125、-2.400、-2.310,均P〈0.05),而睾酮低于左侧(t=0.417,P〉0.05);②右侧卵巢成熟卵率数、正常受精率、可移植胚胎率显著高于左侧(χ^2值分别为5.999、3.923、4.476,均P〈0.05);③卵泡液中白血病抑制因子浓度与正常受精双原核数(2PN)呈正相关趋势(左侧:r=0.3975,P=0.06;右侧:r=0.3892,P=0.06)、与可移植胚胎数呈明显正相关(左侧:r=0.4805,P=0.02;右侧:r=0.3928,P=0.04),而雌二醇、孕激素、睾酮与卵巢减数分裂中期卵子数(MⅡ)无相关性.结论 ①左右侧卵巢卵泡液中细胞因子及内分泌水平存在显著差异,右侧优于左侧;②卵泡液中白血病抑制因子含量可作为评估卵子正常受精及早期胚胎质量的参考指标之一. Objective To study relationship between contents of leukemia inhabitory factor ( LIF)/estrogen ( E2 ), progesterone ( P), testerone (T) in follicle fluid of left and right ovaries and oocyte quantity/quality at in vivo fertilization-embyonic transplantation (IVFET)/intracytoplasm sperm injection (ICSI). Methods Serum and follicle fluid of 37 infertile patients who primarily received IVF/ICSI cycles were taken on the oocyte pick up day ( OPU ). LIF in follicle fluid of left and right ovaries' were detected by using ELISA, and contents of E2 , P and T were detected with chemiluminescence. Results ①LIF, E2 and P contents in follicle fluid of right ovary were significantly higher than follicle fluid of left ovary(t = -2. 125, -2.400, -2.310, all P 〈0.05) , while T conten was lower than the left ovary ( t = 0. 417, P 〉 0.05 ) ; ②Mature oocyte rate/normal fertilization rate/transmitable embryoe rate in the right ovary were higher than the left ovary (χ^2 = 5. 999, 3. 923, 4. 476 respectively, all P 〈 0.05 ) ; ③LIF content in follicle fluid was positively correlated to 2PN number ( the left ovary : r = 0. 3975, P = 0.06 ; the right ovary : r = 0. 3892, P = 0.06 ; P 〈 0.05) and transmitable embryo number( the left ovary: r =0.4805, P =0.02; the right ovary: r =0.3928, P =0.04). However, there were no correlations beween E2 , P, T contents and number of oocyte in meiotic division Ⅱ phase M Ⅲ). Conclusion①There are significant differences between the left ovary and the right ovary in LIF and endocrine hormones' eontens in follicle fluid ; ②LIF may be used as an index to evaluate oocyte normal fertilization and embryo quality in early stage.
