摘要
目的 探索初产前人工流产对再次妊娠妇女健康的影响.方法 采用回顾性队列研究的方法,对2003年5月~11月在武汉市6所医院住院分娩的1 500例单胎初产妇按照有无人工流产史分为人工流产组和非人工流产组,于产后第2天对其进行问卷调查并摘抄住院病历.结果 在前置胎盘、分娩期胎盘并发症(包括胎盘粘连、残留、植入)、第三产程延长以及产后出血比例方面,人工流产组均高于非人工流产组,两组间差异有统计学意义(χ^2值分别为1.2492、10.4513和4.7152,均P〈0.05);经多因素非条件Logistic回归分析显示,人工流产高危行为(包括人工流产史、人工流产次数、末次人工流产与本次妊娠时间间隔)会使第三产程延长的风险增加,OR=1.397(95%CI:1.023~2.178).结论 初产前人工流产对再次妊娠妇女健康的影响较大,应引起足够重视.
Objective To investigate impact of induced abortion before primiparity on health of subsequently pregnant women. Methods In retrospective cohort study method, 1 500 singleton nulliparous parturients admitted to and delivered in 6 hospitals in Wuhan city in a period from May to November, 2003 were selected and divided into two groups: induced abortion group and non induced abortion group according to whether the primipara had history of induced abortion or not. All subjects were investigated with a questionnaire on the second postnatal day and their medical records were abstracted at the same time. Results The incidence rates of placenta previa, placental complication during processes of labor (including retained placenta, placental adhesion and placental implantation), postpartum hemorrhage, prolonged the third stage of labor among primiparae in the induced abortion group were all higher than those of the primiparae in the control group, and the differences were significant (χ^2 = 1. 2492, 10.4513 and 4. 7152 respectively, all P 〈 0.05). Multi-factorial unconditional logistic regression analysis showed that high risk behaviors of induced abortion (including having induced abortion history, frequent induced abortion, short inter-pregnancy interval) increased the risk of prolonged the third stage of labor with OR of 1. 397 (95 % CI:1. 023-2. 178). Conclusion Induced abortion before primiparity pose some serious impacts on health of subsequently pregnant women.
