摘要
目的探讨急性肺血栓栓塞症患者肺实变、肺不张及胸腔积液等肺部征象的变化特点与血清D-二聚体、肌钙蛋白水平变化间的联系。方法观测27例肺血栓栓塞患者肺CT及增强CT影像改变特征与检测血清D-二聚体、肌钙蛋白水平变化的内在相关联系。结果肺血栓栓塞早期肺段或叶远端先变小、突变、继而肺不张,再出现积液,约90%的患者积液量在100ml以内,血性为主,有局部胸膜反应;血清D-二聚体和肌钙蛋白水平变化呈抛物线改变,以在发病2h内达高峰,8h降至正常。结论肺血栓栓塞肺段或肺叶远端先受损,出现肺不张再出现小量积液,栓塞面积大小和严重程度与血清D-二聚体、肌钙蛋白水平变化呈正比,胸腔积液形成后血清D-二聚体、肌钙蛋白水平变化恢复正常。
Objective To Investigate the relationship between imaging features changes of pulmonary consolidation,atelectasis,pleural effusion and serum D-dimer,troponin levels in patients with acute pulmonary thromboembolism.Methods The lung CT and enhanced CT images to change features were observed in 27 patients with pulmonary thromboembolism and detection the inherent relationship with serum D-dimer,troponin levels.Results In pulmonary thromboembolism,distal lung segment or lobe lessensed at first,then mutation,followed by atelectasis,at last of effusion.About 90% of patients within the fluid volume of 100ml,bloody-based,with local pleural reaction.Serum D-dimer and cardiac troponin I levels presented in a parabola changes,presented to a peaked value within 2 hours,recovered to normal within 8 hours after disease onset.Conclusion In pulmonary thromboembolism,distal lung segment or lobe damage first,then appears atelectasis,and again a small amount of effusion.The thromboembolism size and severity have positive association with serum D-dimer,troponin levels.After the formation of pleural effusion,the serum D-dimer and troponin levels return to normal.
