内镜窄带成像技术对大肠肿瘤诊断价值的研究 被引量:1

摘要 目的探讨内镜窄带成像技术(Narrow-Band Imaging,NBI)对大肠肿瘤的诊断价值。方法选择2008.5-2008.12间共981例患者进行常规内镜检查,其中发现大肠新生物或息肉样病变者共334例入选。分别采用常规放大技术、NBI放大技术及染色放大技术对病变作出诊断,并与病理组织学诊断相比较。结果在334例病人中发现新生性病变676个,其中常规放大内镜下发现病变91.1%(616/676),NBI放大模式下发现病变99.1%(670/676),差异有统计学意义。NBI放大内镜对肿瘤性及非肿瘤性病变判断的符合率为91.6%(619/676),染色放大内镜符合率为82.0%(555/676),前者高于后者(P<0.05)。结论 NBI放大内镜对大肠肿瘤性及非肿瘤性病变诊断的符合率高,有良好的临床实用价值。 [Objective] To Study the diagnostic efficacies of Narrow-Band Imaging for the differential dignosis of colon tumor and non-tumor lesion.[Methods] Nine hundred and eighty one patients were examined by conventional colonoscopy in the Yu Huang Ding Hospital from May to November in 2008.Three hundred and thirty four patients were enrolled in this study.They were observed with conventional colonscopy,and NBI and chromoendoscopy.Lesions were resected or biopsied for histopathological analysis.Sensitivity,specificity,and diagnostic accuracy of each endoscopic modality were assessed by reference to histopathology.[Results] Sixty hundred and seventy six colorectal lesions were found in 981 patients.616 lesions were found under the conventional colonoscopy,while 60 flat lesions were missed.Only 6 flat lesion were missed under NBI.The detection rate of conventional colonscopy and NBI was 91.1% and 99.1%.respectively(P0.01).The accuracy of NBI and chromoendoscopy was 91.6% and 82.0% respectively(P0.05).[Conclusions] NBI were capable for distinguishing neoplastic from non-neoplastic colorectal lesions and the diagnostic accuracy of NBI was better than that of conventional colonscopy and equivalent to that of chromoendoscopy.It is an effective technology to reduce the time of examination.
