摘要
目的:探讨MRI在婴幼儿脑性瘫痪(简称脑瘫)诊断中的价值。材料与方法:回顾性研究232例婴幼儿脑瘫患儿的临床及MRI影像表现。结果:232例脑瘫患儿中痉挛型165例(四肢瘫84例,偏瘫47例,双瘫32例、三瘫2例),手足徐动型21例;共济失调型6例,肌张力减低型13例,混合型27例。MRI显示异常197例(84.9%),其中脑室周围白质软化症(PVL)99例,脑软化、萎缩20例,基底节及丘脑病变14例,髓鞘形成不良44例,脑积水11例,脑发育不良19例,脑发育畸形19例。MRI影像表现中最常见的是PVL(42.7%),临床主要引起痉挛性瘫痪。另外脑瘫早产儿组较足月儿组PVL发生率更高,两者差异具有统计学意义(p<0.05)。结论:MRI在婴幼儿脑瘫早期诊断、临床病因分析、预后判断等方面有重要价值。
Purpose：To explorer the diagnostic values of MRI in infant cerebral palsy（cp）.Materials and Meth-ods：The outcome of clinic and MRI are retrospective study in 35 infant cerebral palsy.Results：232 cases of cp included spastic CP in 165（84 with tetraplegia,47 with hemiplegia,32 with spastic diplegia,2 with triplegia）,dys-kinetic CP in 21,ataxic cp in 6,hypotonic CP in 13 and mixed CP in 27.MRI abnormalities were identi？ ed in 197 of the 232 cases.（84.9%）,including periventricular leukomalacia（pvl） in 99,cerebromalacia and encephalatrophy in 20,the lesion of bas-al ganglia and thalamic brain in 14,the brain cacoethic myelination in 44,hydrocephalus in 11,brain hypogenesis in 19,brain malformations in 19.The most common abnormalities identi？ ed on MRI were pvl（42.7%）,which mainly gives rise to spastic CP.In addition,There was significant difference in the rates of pvl between term and preterm groups（p 0.05）.The rates of pvl among term groups is higher than that among preterm groupsConclusion：MRI is playing an important role in the early diagnosis Clincal etiology analysis and the prognosis of cerebral palsy in infant
