摘要
目的研制具有中医内涵的斑秃患者生活质量量表,全面、客观的评价中医药治疗斑秃的临床疗效。方法根据WHO生活质量量表制定原则,结合中医基础理论,提出量表的理论构想,广泛查阅国内外多种普遍性及皮肤病专用量表,广泛听取议题小组、专家和斑秃患者的意见,采用结构化决策的方法,构建初始量表。以访谈式测量120例斑秃患者的中医生活质量及患者的一般情况,采用频数分布、离散趋势法、相关系数法、因子分析法来进行统计筛选后最终形成斑秃生活质量量表。结果研制出生理、心理、社会3个维度共29个条目组成斑秃中医生活质量量表,与理论构想相符。量表24 h重测信度为0.996,Cronbach'α系数为0.927,分半信度系数为0.933。因子分析提取了7个公因式,累计方差贡献率73.815%,第一公因式为斑秃对日常生活能力的影响,方差贡献率20.540%。量表总分与SF-36总分、DLQI总分相关系数分别为0.779、0.762。患者中医药治疗前后量表总分差异有统计学意义,不同性别组、不同文化程度组、不同年龄组的患者斑秃生活质量量表总分差异均有统计学意义。结论该量表有较理想的信度、效度、反应度及可行性,同时具有中医特色,准确反映了斑秃患者生活质量的内涵,可作为斑秃中医临床疗效评价的工具。
Objective To develop a life quality scale for patients with alopecia areatain TCM and evaluate clinical effect of Chinese medicine.Methods Based on the life quality of definition from WHO and theory of TCM,120 patients with alopecia areata were interviewed in their life quality and general conditions,the frequency distribution,dispersion analy-sis,correlation analysis and factor analysis were used to analyze,and then,to form the alopecia areata quality of life scale（AAQOLS）.Result AAQOLS with 29-item and 3 domins including physical,psycholoqical and sociaty was developed for alopecia areata patients.It was consistent with the theory construction.The test-retest correlation coefficient,cronbach＇s a corfficient and split-half reliability coefficient of AAQOLS were 0.996,0.927,and 0.933.The scale＇s correlation with SF-36,DLQI score were 0.779,0.762.AAQOLS can well discriminate among those with different quality of lives.Conclusion AAQOLS is reliable effective,sensitive and with TCM feature,can interprete the connotation of alopecia areata＇s quality of accurately,which can be used to evaluate clinical curative effect with TCM.
