中老年血液透析患者颈总动脉僵硬度的影响因素 被引量:1

Determinants of arterial distensibility in middle-age and elderly patients on the treatment of hemodialysis

摘要 目的 评价中老年血液透析患者颈总动脉僵硬度,分析可能影响颈总动脉僵硬度的危险因素.方法 采用超声技术测定本透析中心87例中老年血液透析患者颈总动脉可扩张性系数(DC)作为评价颈总动脉僵硬度的指标,常规检测白蛋白、总胆固醇、高密度脂蛋白、低密度脂蛋白、三酰甘油、血糖、肌酐、钙、磷、甲状旁腺素.采用直线相关、多因素逐步回归分析影响颈总动脉僵硬度相关的独立因素.结果 本研究的87例患者,年龄45~81岁,(60.18±9.67)岁,透析龄3~204个月.颈总动脉DC水平为(13.39±5.32)×10^-3/kPa,多元逐步回归分析显示收缩压(β=-0.349,P<0.001)、年龄(β=-0.323,P<0.001)、血钙(β=-0.276,P=0.002)、糖尿病(β=-0.195,P=0.027)与颈总动脉DC独立相关.结论 收缩压、年龄、血钙升高和糖尿病是中老年血液透析患者颈总动脉僵硬度增加的独立危险因素. Objective To evaluate arterial stiffness in middle-age and elderly patients on the treatment of hemodialysis and to investigate the risk factors of arterial stiffness. Method A total of 87 in middle-age and elderly patients on the treatment of hemodialysis were enrolled. The distensibility coefficient (DC) of the common carotid artery was evaluated by an ultrasonic phase-locked echo-tracking system. Serum albumin, total cholesterol (TC), high density lipoprotein (HDL), low density lipoprotein (LDL), triglyceride (TG), glucose, creatinine,calcium, phosphorus, and intact parathyroid hormone (iPTH) were determined with well established methods. Results The ages of 87 patients ranged from 45 to 81 years, and the duration of treatment with hemodialysis was varied from 3 to 204 months . The carotid DC was ( 13.39 ± 5.32 ) × 10^-3/kPa in middle-age and elderly patients on hemodialysis. In stepwise multiple regression analysis, systolic blood pressure (β = -0.349, P 〈 0.001), age (β=-0.323, P 〈 0.001), diabetes (β =-0.195,P = 0.027), and serum calcium (β =-0.276,P =0.002) were independently correlated with carotid DC. Conclusions Systolic blood pressure, age, diabetic,serum calcium were independent determinants for arterial stiffness of carotid artery in middle-age and elderly patients on hemodialysis.
