摘要
目的:观察胸腔内中心静脉导管引流并灌注治疗高龄肺癌恶性胸腔积液疗效和不良反应。方法:2007年7月-2009年7月,对46例高龄肺癌恶性胸腔积液患者应用中心静脉导管胸腔引流,并随机分为A组(23例)和B组(23例)。A组予生理盐水60ml加卡铂400mg胸腔灌注,每周1次,共2次;B组予鸦胆子油乳60ml胸腔内灌注,每周2次,共4次。比较两组治疗前、后胸水的变化、胸水中癌胚抗原(CEA)浓度变化、KS评分和不良反应。结果:A组、B组有效率分别为86.9%和60.9%,差异有统计学意义(P<0.05),两组治疗后Karnofsky评分升高率分别为52.2%和43.5%,无统计学意义。两组治疗后胸水中CEA浓度较治疗前明显下降(P<0.05),A组较B组明显降低(P<0.05),A组骨髓抑制较B组严重,其余不良反应比较无统计学意义(P>0.05)。结论:胸腔内置管引流并灌注治疗肺癌恶性胸腔积液的方法简便易行、创伤小、安全,卡铂疗效优于鸦胆子油乳。
Objective：To explore the methods and effect of intracavity drainage plus perfusion medication on treating malignant pleural effusion caused by lung cancer.Methods：From July 2007 to July 2009,forty six elder patients with malignant pleural effusion caused by lung cancer were randomly divided into A group（23 cases） and B group（23 cases）.Intrapleural injection of carboplatin was administered in A group,and Brucea javanica oil emulsion（BJOE） was administered in B group after drainage of pleural effusion.The effect on malignant pleural efusion,the change of concentration of earcino-embryonic antigen（CEA） and the side effect were compared before and after treatment.Results：A group achieved total response rate of 86.9%,but only 60.9% in B group,with significant difference（P 0.05）.Furthermore,the concentration of CEA in A group dramatically decreased comparing with those of B group（P 0.05）.But the common toxicity was leucopenia in A group and the difference was significant compared with that in B group,there was no significant difference in other side effect（P 0.05）.Conclusion：Intracavity drainage plus perfusion medication has advantage in the treatment of malignant pleural effusions by lung cancer.
