摘要
目的:观察EP方案同步放化疗治疗局限期小细胞肺癌疗效及相关不良反应。方法:58例病理或细胞学证实小细胞肺癌,根据美国退伍军人医院肺癌研究组分期标准均为局限期,随机分为EP方案同步放化疗组和序贯放化疗组。同步组DDP 60mg/m2d1-2,Vp-16 100mg/d,d1-4。放疗与第一周期化疗同时进行。放疗结束继续化疗四周期,序贯组EP方案DDP 60mg/m2d1-2,Vp-16 100mg/d,d1-4,4周期化疗后,开始放疗,放疗结束继续2周期化疗。放疗均采用6MV-X,三维适形放疗。95%PTV总剂量60Gy/30次/6周。比较两组不良反应,近期疗效,1-3年生存率、1-3年局控率。结果:EP方案同步放化疗组近期完全缓解率(CR)76%,而序贯组为55%,差异有显著性(P<0.05)。1-3年生存率同步放化疗组79.3%、62.1%、41.4%,序贯组55.2%、38%、20.7%。1-3年局控率同步组95%、82%、63%,序贯组73%、56%、32%。3年远处转移同步组50%,序贯组65%。根据WHO CTC3.0评价近期不良反应,两组无显著差异。结论:EP方案同步放化治疗局限期小细胞肺癌,放疗采用三维适形,局部控制较好,1-3年生存率较序贯放化疗组提高。且不良反应无明显增加。远处转移同步组较序贯组下降。
Objective：To study the responses of EP regimen concurrent chemoradiotherapy for loclized small-cell lung caner（SCLC）.Methods：Total of 58 patients with limited SCLC were randomized into two groups（n = 29）,one group was given sequential chemoradio-chemotherapy（A group）,and the other group was given concurrent chemoradiotherapy（B group）,all paitents received EP regimen chemotherapy for 6 cycles.In group A radiotherpy was given after the fourth chemotherapy cycle;and in group B radiotherapy was delivered with the first cycle chemotherapy concurrently,all patients were treated with 3D radiotherapy and 95% PTV received a prescription dose of 60Gy/6 weeks,2Gy/fraetion.Then the side effects response and survival rate of 1-3 year and local control rate of 1-3 year were compared.Results：The complete response rate in guoup A and B were 55% and 76%,respectively（P 0.05）.The 1,2,3 year survival rates of guoup A and B were 55.2%,38%,20.7% ;79.3%,62%,41.4%.The local control rates of 1,2,3 year were A group 73%,56%,32%,B group 95%,82%,63%,the 3-year distant metastasis rates of gruop A and B were 65% and 50% respectively.Conclusion：For localized small-cell lung cancer the concurrent chemoradiotherapy is better than the sequential treatment.The toxic effect of conceurrent therapy is acceptable.
