摘要
目的:评价国内恶性脑胶质瘤术后放疗联合替莫唑胺化疗的疗效与安全性。方法:计算机检索中国期刊全文数据库CNKI(1999-2009)、中国生物医学文献数据库CBM disc(1999年-2009年)、中文科技期刊全文数据库VIP(1999年-2009年),手工检索所有纳入文献的参考文献。纳入术后放疗联合替莫唑胺治疗恶性脑胶质瘤的随机对照试验(RCT)。评价纳入研究的方法学质量并进行资料提取后,采用RevMan4.2.10软件进行Meta分析。结果:共纳入5个RCT,包括241例患者。Meta分析结果显示,术后放疗联合替莫唑胺化疗组与单纯术后放疗组比较,其总有效[RR=2.28,95%CI(1.65,3.15)]和1年生存率[RR=1.87,95%CI(1.47,2.38)]、2年生存率[RR=2.61,95%CI(1.71,3.99)]差异均有统计学意义。结论:目前国内的有限证据表明,与术后单纯放疗相比,术后放疗联合替莫唑胺化疗能提高恶性脑胶质瘤的总有效率和1年生存率、2年生存率,但由于纳入研究样本量小且质量较低,上述结论尚需要高质量、大样本的随机双盲对照试验加以证实。
Objective：To assess the effectiveness and safety of postoperative radiotherapy plus temozolomide for treating malignant glioma in China.Methods：Randomized controlled trials（RCTs） of postoperative radiotherapy plus temozolomide for malignant glioma were collected from CNKI（1999 to 2009） CBMdisc（1999 to 2009）,and VIP（1999 to 2009） databases.Other relevant journals were also handsearched.The methodological quality of the included studies was evaluated,and data analyses were performed using Cochrane Collaboration＇s software RevMan 4.2.10.Results：A total of 5 RCTs involving 241 patients were included.As for the total effective rate and the 1-,2-year survival rate,significant differences were found between postoperative radiotherapy plus temozolomide vs.postoperative radiotherapy alone（RR = 2.28,95% CI 1.65 to 3.15 and RR = 1.87,95% CI 1.47 to 2.38;RR = 2.61,95% CI 1.16 to 3.99;RR = 3.06）.Conclusion：According to the domestic evidence,treatment of malignant glioma with postoperative radiotherapy plus temozolomide can improve effective rate and 1-,2-year survival rate.However more high-quality,large-sample,randomized,double-blind,controlled trials are required.
