摘要
目的探讨外周血淋巴细胞表面标志物CD69在新生儿感染性疾病诊断中的价值。方法选取2007年3月-2008年12月在新生儿科(包括NICU)住院的患儿82例,分为重症感染组(42例)、轻症感染组(18例)和非感染组(22例)。应用流式细胞仪测定各组患儿外周血淋巴细胞CD69表达水平。应用SPSS 10.0软件进行统计学分析。结果非感染组、轻症感染组、重症组患儿外周血淋巴细胞CD69表达百分比分别为(1.13±0.69)%、(2.47±1.03)%、(2.16±1.24)%。轻症感染组患儿外周血淋巴细胞CD69表达水平显著高于非感染组(P=0.003),重症感染组患儿外周血淋巴细胞CD69表达水平显著高于非感染组(P=0.017),轻症感染组与重症感染组比较差异无统计学意义(P=0.546)。接受者操作特性曲线(ROC)计算得出CD69取临界值为2.19%,诊断新生儿感染的敏感度为42%,特异度为100%;CD69取临界值为1.29%,敏感度为83%,特异度为56%;CD69取临界值为0.99%,敏感度为92%,特异度为56%。结论新生儿感染时外周血淋巴细胞CD69表达水平明显升高,可作为早期诊断新生儿感染的实验室依据之一。
Objective To explore the diagnostic value of peripheral blood lymphocyte surface marker CD69 expression for neonatal infectious disease.Methods From Mar.2007 to Dec.2008,82 neonatal patients(including NICU patients) were divided into 3 groups:sepsic infection group(42 sepsic cases),mild infection group(18 mild infectious cases) and non-infection group(22 non-infection cases).The le-vels of lymphocyte CD69 were checked with flow cytometry.SPSS 10.0 software was used to analyze these data.Results The percentage of CD69 expression was(1.13±0.69)% in non-infection group,(2.47±1.03)% in mild infection group,(2.16±1.24)% in sepsic infection group.The percentage of CD69 expression in mild infection group was significantly higher than that in non-infection group(P=0.003),and that in sepsic infection group was significantly higher than that in non-infection group(P=0.017).There was no significant difference between sepsic infection group and mild infection group(P=0.546).Receiver operating characteristic(ROC) curve was used to calculate the critical value.When the critical value of CD69 was 2.19%,the sensitivity of neonatal sepsis was 42% and the specificity was 100%;when the critical value of CD69 was 1.29%,the sensitivity was 83%,and specificity was 56%;when the critical value of CD69 was 0.99%,the sensitivity was 92% and the specificity was 56%.Conclusion Peripheral blood lymphocyte increase significantly in infectious diseases,and CD69 expre-ssion is a good laboratory mark to early diagnosis of neonatal infection.
