巨细胞病毒感染婴儿T辅助细胞分化及转录因子的调控作用 被引量:5

Regulative Role of T Cell-Specific Transcription Factors and T Helper Cell Differentiation in Infants with Human Cytomegalovirus Infection

摘要 目的探讨人巨细胞病毒(HCMV)感染婴儿Th1/Th2细胞分化情况及转录因子T-bet/GATA-3的调控作用。方法收集2006年11月-2008年6月HCMV感染患儿36例(病例组),同期健康体检儿童36例作为健康对照组。使用淋巴细胞分离液分离淋巴细胞,采用ELISA法测定其外周血淋巴细胞培养上清液中细胞因子γ干扰素(IFN-γ)及IL-10水平,直接免疫荧光标记法测定其淋巴细胞C-C趋化因子受体(CCR3+、CCR5+)阳性细胞率,实时荧光定量PCR法检测其淋巴细胞中转录因子T-betmRNA、GATA-3mRNA表达强度。结果病例组IFN-γ水平[(652.8±204.4)ng.L-1]低于健康对照组[(1 483.6±417.2)ng.L-1],IL-10水平[(31.1±12.5)ng.L-1]高于健康对照组[(11.3±5.7)ng.L-1],CCR5+细胞比例(5.9%)低于健康对照组(12.2%),CCR3+细胞比例(7.5%)高于健康对照组(3.8%),T-betmRNA表达水平(0.182 5±0.036 6)低于健康对照组(0.260 3±0.041 5),GATA-3mRNA(0.402 7±0.079 1)则高于健康对照组(0.285 2±0.051 7)(Pa<0.05,0.01)。病例组IFN-γ水平与T-betmRNA水平呈正相关(r=0.681,P<0.05),IL-10水平与GATA-3 mRNA水平呈正相关(r=0.705,P<0.05),IFN-γ/IL-10与T-bet/GATA-3呈正相关(r=0.863,P<0.05)。结论 1.HCMV感染婴儿IL-10水平增高,IFN-γ水平降低,CCR3+表达增高,CCR5+表达降低,存在以Th2细胞优势分化状态为特征的T辅助细胞分化失衡。2.HCMV感染婴儿Th1/Th2细胞分化失衡受核转录因子T-bet和GATA-3的调控。实用儿科临床杂志,2010,25(22) Objective To explore the Th1/Th2 cell differentiation in infants with human cytomegalovirus(HCMV) infection and the healthy modulation of T cell-specific transcription factors T-bet(T-box expressed in T cells) and GATA-3.Methods Thirty-six HCMV infected infants were enrolled as HCMV group from Nov.2006 to Jun.2008,and 36 healthy children were selected as healthy control group at the same time.The lymphocytes were isolated by lydroxypropylmethyl cellulose.Interferon γ(IFN-γ)and IL-10 concentrations in the supernatant were detected by enzyme linked immunosorbent assay.The T-bet mRNA and GATA-3 mRNA expression levels in lymphocytes were assayed by real-time reverse transcription polymerase chain reaction.The ratio of CCR3+ and CCR5+ cells in lymphocytes were counted by flow cytometry after direct immunofluorescence staining.Results IFN-γ concentrations and the ratio of CCR5+ cells in HCMV group were(652.8±204.4) ng·L-1 and 5.9%,which were significantly lower than those in healthy control group[(1 483.6±417.2)ng·L-1 and 12.2%](P0.01,0.05).IL-10 concentrations and the ratio of CCR3+ cells in HCMV group were(31.1±12.5) ng·L-1 and 7.5%,which were significantly higher than those in healthy control group[(11.3±5.7)ng·L-1 and 3.8% ](P0.01,0.05).The level of T-bet mRNA expression in HCMV group was 0.182 5±0.036 6,which was significantly lower than that in healthy control group(0.260 3±0.041 5)(P0.01).The level of GATA-3 mRNA expression in HCMV group was 0.402 7±0.079 1,which was significantly higher than that in healthy control group(0.285 2±0.051 7)(P0.01).A positive correlation was found between IFN-γ concentration and T-bet mRNA expression level(r=0.681,P0.05).IL-10 concentration was positively correlated with GATA-3 mRNA expression level(r=0.705,P0.05) and IFN-γ/IL-10 was positively correlated with T-bet/GATA-3(r=0.863,P0.05).Conclusions 1.In HCMV infection,there is a tendency of Th2 polarization with over-production of Th2-like cytokines and CCR3+ cells.2.The imbalance of T helper cell differentiation is modulated by T cell-specific transcription factors T-bet and GATA-3.
