摘要
目的:评价正电子发射断层显像(positron emission tomography/computed tomography,PET/CT)诊断肺癌的准确度,分析其证实偏倚校正后的灵敏度及特异度,为临床客观而全面地认识PET/CT诊断的结果提供依据。材料和方法:用最大似然估计值法对PET/CT诊断肺癌的灵敏度和特异度进行校正。结果:传统有偏估计的灵敏度为94.21%(95%CI:90.89%~97.53%),校正证实偏倚后的灵敏度为87.07%(95%CI:81.36%~92.78%),两者95%可信区间有交集,其差异无统计学意义;传统有偏估计的特异度为44.07%(95%CI:31.39%~56.73%),校正证实偏倚后的特异度为65.57%(95%CI:56.82%~74.31%),两者95%可信区间无交集,其差异具有统计学意义,证实偏倚校正后的特异度高于校正前。结论:PET/CT诊断肺癌的灵敏度较高。证实偏倚的存在,影响对PET/CT诊断肺癌的准确度评价。
Purpose To estimate the accuracy of PET/CT for lung cancer and analyze the sensitivity and specificity after the verification of bias correction.To provide the basis for clinical understanding of PET/CT diagnostic results objectively and comprehensively.Materials and Methods Correct the sensitivity and specificity of lung cancer diagnosed by PET/CT using the methods of maximum likelihood estimator.Results The sensitivity of lung caner diagnosed by PET/CT before and after the verification bias been corrected were 94.21%(95%CI:90.89%-97.53%)and 87.07%(95%CI:81.36%-92.78%).The 95%confidence intervals of the sensitivity had intersection,and there were no statistical differences between them.The specificity was increased from 44.07%(95%CI:31.39%-56.73%)to 65.57%(95%CI:56.82%-74.31%)after the verification bias been corrected at the same time,The 95%confidence intervals of them had no intersection,and there were statistical differences between them.Conclusion PET/CT has higher sensitivity for lung cancer.Verification bias can influences on the accuracy of PET/CT in diagnosing lung cancer.
