冻融胚胎移植周期患者年龄及移植胚胎个数与妊娠结局的关系 被引量:6

Relationship between Pregnancy Outcome and Maternal Age and Number of Transferred Embryos in Infertile Patients Undergoing FET

摘要 目的:探讨冻融胚胎移植周期患者的年龄、移植胚胎个数对妊娠结局的影响。方法:解冻胚胎移植周期中可供移植的1052个周期按年龄分为≤34岁为A组、35~39岁为B组、≥40岁为C组,按移植胚胎个数分为单胚胎移植为D组、2个胚胎移植为E组、3个胚胎移植为F组,分析年龄、移植胚胎个数与冷冻胚胎解冻移植(FET)结局的关系。结果:A组、B组、C组间着床率差异有统计学意义(P<0.01)。A组与B组妊娠率差异无统计学意义(P>0.05),但均高于C组(P<0.05)。A组与B组流产率比较差异有统计学意义(P<0.05)。D组妊娠率与E、F组比较差异有统计学意义(P<0.05)。E组多胎率与F组比较差异有统计学意义(P<0.05)。E组减胎率与F组比较差异有统计学意义(P<0.05)。D、E、F组间宫外孕率差异均无统计学意义(P>0.05)。结论:≥40岁患者妊娠率显著降低。宫外孕、流产与移植胚胎个数无关。冻融周期移植2个胚胎是合理的选择。 Objective: To investigate the influence of maternal age and the number of transferred embryos on pregnancy outcomes in infertile patients undergoing frozen thawed embryo transfer (FET). Methods: A retrospective analysis was performed in our center on the clinical data of 1 052 thawing cases from January 2004 to December 2008. There were 6 groups in the study including A group (≤34 year-old), B group (35-39 year-old), C group (≥40 year-old), D group(single embryo transfer),E group (2-embryo transfer ) and F group(3-embryo transfer). Results: The implantation rate was significantly lower in A group compared with that of B group (P 0.05), but higher than that of C group (P 0.05). The abortion rate was significantly lower in A group compared with that of B group (P 0.05).There were significant differences in multiple pregnancy rate and fetal reduction rate between E group and F group (P 0.05). There was no difference in ectopic pregnancy rate between groups (P 0.05). Conclusion: The implantation rate decreased significantly in patients≥40 year-old. The number of transferred embryos was not correlated with ectopic pregnancy and abortion rate. Transplantation of 2 call embryos is the reasonable choice.
