摘要
目的探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者中脾虚证型与代谢综合征(metabolic syndrome,MS)的相互关系,并分析两者关联的内在病理机制。方法收集102例PCOS患者,检测空腹血糖(FBG)、胰岛素(FINS)等相关指标;测量腰围、体质量、身高及血压。根据中医辨证分型,分成脾虚组(50例)与非脾虚组(52例);按照MS的诊断标准,分成MS组(22例)与非MS组(80例)。对患者的临床资料和实验室数据进行比较分析。结果 MS组血清FINS水平与胰岛素抵抗指数(HOMA-IR)显著高于非MS组(P<0.01);脾虚组FINS水平与HOMA-IR显著高于非脾虚组(P<0.01);MS、脾虚与FINS水平、HOMA-IR水平皆呈显著正相关(P<0.01);脾虚组的MS患病率显著高于非脾虚组(P<0.05);脾虚与MS呈正相关(P<0.05),但在控制FINS或HOMA-IR因素后两者相关性比较差异无统计学意义(P>0.05)。结论脾虚证型PCOS患者是MS的高危人群,也是MS低龄化不可忽略的因素,其原因可能和胰岛素抵抗有关;及早诊治PCOS,尤其对脾虚型患者,对于防止MS等并发症有重要意义。
Objective To evaluate the relation between Pi-deficiency syndrome（PDS） pattern and metabolic syndrome（MS） in patients with polycystic ovarian syndrome（PCOS）,for exploring their internal pathologic mechanism.Methods Among the 102 PCOS patients,22 complicated with MS（PCOS-MS） and 80 not complicated with MS（PCOS-NMS）,the Chinese medicine syndrome pattern was differentiated as PDS in 50 patients and non-PDS in 52.The clinical data,in terms of fasting blood glucose（FBG）,fasting insulin（FINS）,waistline,body weight（BW）,stature,blood pressure（BP）,etc.was collected and compared and the relation between data was analyzed.Results Levels of FINS and homeostasis model of assessment for insulin resistence index（HOMA-IR）,in PCOS-MS patients were significantly higher than those in PCOS-NMS patients,also higher in patients of PDS pattern than those of non-PDS pattern（P0.01）;the occurrences of MS and PDS were highly positively correlated with levels of FINS and HOMA-IR（P0.01）;incidence of MS in patients of PDS pattern was significantly higher than those in patients of non-PDS pattern（P0.05）;presenting of PDS was positively related with the existence of MS（P0.05）,but in case of the FINS or HOMA-IR factor being controlled,statistical meaning of the relativity between them turned to insignificant（P0.05）.Conclusions PCOS patients of PDS pattern are the high-risk population of MS,which might be related with the insulin resistance.So,early treatment of PCOS,especially on patients of PDS pattern,is of important significance for preventing the complication,as MS,of the disease.
