摘要
目的探讨8种自身抗体联合检测在自身免疫性疾病诊断中的临床意义。方法自身免疫性疾病患者145例(A组),健康体检者100例(B组)。采用间接免疫荧光法、斑点法、双抗原夹心酶联免疫吸附法联合检测抗dsDNA、抗Sm、抗U1RNP、抗SSA、抗SSB、抗Jo-1、抗Scl-70及抗CCP等8种自身抗体,并对检测结果进行统计学分析。结果各类风湿病患者均有阳性出现,总检出率88.3%,其中以女性30～50岁年龄段检出率较高。检出一项或一项以上自身抗体阳性者:系统性红斑狼疮(SLE)93.1%(27/29)、多发性肌炎/皮肌炎(PM/DM)80.%(8/10)、进展期硬皮病(PSS)83.3%(5/6)、干燥综合征(SS)94.1%(32/34)、混合性结缔组织病(MCTD)100%(10/10)、类风湿关节炎(RA)80.0%(44/55),阳性率较高。结论自身抗体8项联合检测可以提高对自身免疫性疾病诊断的特异性,有助于早期自身免疫病的诊治。
Objective To study the clinical significance of combined detection of eight autoantibodies in diagnosis of autoimmune diseases.Methods Using indirect immunoflurescence（IIF） assay,dot-immunobinding assay and double antigens sandwich ELISA,eight autoantibodies including anti-dsDNA,anti-Sm,anti-U1RNP,anti-SSA,anti-SSB,anti-Jo-1,anti-Scl-70 and anti-CCP were detected in 145 patients with autoimmune diseases（group A） and 100 healthy persons（group B）.Results All rheumatisants had positive results with a total detection rate of 88.3%,which was higher in the women aged 30 to 50 years old.The positive rate with one or more autoantibodies was 93.1% in SLE,80% in PM/DM,100% in PSS,94.1% in SS,100% in MCTD and 80% in RA.ConclutionThe combined detection of eight autoantibodies can increase the sepecificity of autoimmune disease and help the early diagnosis and treatment of autoimmune diseases.
